Evaluación de la aplicabilidad terapéutica en pacientes con cáncer colorrectal según su clasificación molecular. Hospital de Clínicas, 2019 - 2023

Resumen:
El cáncer colorrectal (CCR) es una neoplasia maligna compleja con fuerte interacción entre factores ambientales y alteraciones moleculares. A nivel mundial, es el segundo cáncer más frecuente. Uruguay tiene la mayor incidencia en Latinoamérica, lo cual evidencia un importante problema de salud pública debido a la gran carga económica que impone su abordaje. El avance tecnológico y el estudio sobre la genética del CCR permitió elaborar en el año 2015 el consenso de sub clasificación molecular del CCR (CMS) en base a sus características genéticas y epigenéticas estableciendo cuatro subgrupos, permitiendo tratamientos individualizados para mejorar la sobrevida de los pacientes. Objetivo: Analizar la terapéutica oncoespecífica aplicada en los pacientes con cáncer colorrectal del Hospital de Clínicas y si se aplica teniendo en cuenta la clasificación molecular en el periodo enero 2019 - julio 2023. Materiales y métodos: Se llevó a cabo una búsqueda en historias clínicas registradas en bases de datos electrónicas y se procedió al análisis de los mismos a través del Software Epi Info, análisis estadístico “SISA”, “Sample Size Calculators” y Microsoft Excel. Resultados: Se recabó información de 107 pacientes, de los cuales se incluyeron 53. En cuanto a las variables grupo molecular y tratamiento dirigido, del total de la muestra, 19 (35,85%) obtuvo un tratamiento dirigido según el grupo molecular. Conclusiones: Los resultados obtenidos en la investigación en cuanto al tratamiento dirigido constituyen una cifra significativa considerando la reciente implementación de la clasificación molecular, siendo fundamental continuar avanzando en esta línea para que se pueda lograr una mayor aplicación a futuro.
Colorectal cancer (CRC) is a complex malignant neoplasm with a strong interaction between environmental factors and molecular alterations. Worldwide, it is the second most common cancer. Uruguay has the highest incidence in Latin America, which shows an important public health problem due to the great economic burden that its approach imposes. The technological advance and the study on the genetics of CRC made it possible to develop in 2015 the Consensus Molecular Subtypes (CMS), based on their genetic and epigenetic characteristics, establishing four subgroups, allowing individualized treatments to improve patient survival. Objective: Analyze the oncospecific therapy applied to patients with colorectal cancer at Hospital de Clínicas and whether it is applied considering the molecular classification in the period January 2019 - July 2023. Materials and methods: A search was made in clinical records registered in electronic databases and they were analyzed using Epi Info Software, “SISA” statistical analysis, “Sample Size Calculators” and Microsoft Excel. Results: Information was collected from 107 patients, of which 53 were included. Regarding the variables molecular group and targeted treatment, of the total sample, 19 (35.85%) obtained targeted treatment according to the molecular group. Conclusions: The results obtained in the research regarding targeted treatment constitute a significant figure, considering the recent implementation of molecular classification, and it is essential to continue advancing in this direction so that greater application can be achieved in the future.
