Rol de los transportadores ABCB1, ABCC4 y ABCG2 en los niveles plasmáticos de Metotrexato en pacientes pediátricos con Leucemia Linfoblástica Aguda

Resumen:
Las leucemias constituyen las neoplasias más frecuentes en niños, siendo la leucemia linfoblástica aguda (LLA) la más frecuente a nivel mundial así como en Uruguay. El tratamiento de LLA en Uruguay sigue el protocolo BFM que incluye, entre otros fármacos, el metotrexato (MTX). Este puede causar reacciones adversas como hepatotoxicidad y mucositis, entre otras. Este estudio analizó la relación entre los polimorfismos rs2032582, rs9282564 del gen ABCB1, rs3765534 y rs11568658 del gen ABCC4 y rs2231137 del gen ABCG2 y los niveles plasmáticos de MTX 48 horas post-infusión durante la fase de consolidación en 204 pacientes pediátricos con LLA. La normalidad y la homogeneidad de varianzas de las variables fueron analizadas mediante el test Shapiro-Wilk y test Bartlett, respectivamente. La relación entre los distintos genotipos y la variable discreta toxicidad por MTX se analizó mediante el test de chi cuadrado. A su vez, la relación entre las variantes genéticas y la variable continua concentración de MTX a las 48hrs se analizó utilizando los test no paramétricos Mann-Whitney y Kruskal-Wallis, según corresponda. Los resultados encontrados no evidenciaron una asociación significativa entre los distintos polimorfismos y la presencia o ausencia de toxicidad por MTX en ninguno de los cuatro pulsos. Sin embargo, se encontró una relación significativa entre la variante rs2231137 (ABCG2) y la toxicidad por MTX en el pulso total. Por otro lado, en el subgrupo de pacientes que recibieron 2 g/m2, se observó una relación significativa entre la variante en cuestión y el pulso total. Teniendo en cuenta la variabilidad entre pacientes en la eliminación de MTX sería conveniente ampliar este estudio a una muestra mayor, analizar otras variantes de estos genes así como otros genes involucrados en los procesos de absorción, distribución y eliminación, como los genes involucrados en el transporte al interior de la célula de MTX y en su metabolismo. Adicionalmente otra aproximación podría ser estudiar el clearance a nivel renal como indicador de toxicidad.
Acute leukemia’s are the most common neoplasms in children, being Acute Lymphoblastic Leukemia (ALL) the most common worldwide as well as in Uruguay. ALL treatment in Uruguay follows the BFM protocol that includes, among other drugs, methotrexate (MTX). This drug can cause adverse reactions as hepatotoxicity and mucositis, among others. This investigation analyzed the relationship between certain genetic polymorphisms (rs2032582, rs9282564 of the ABCB1 gene, rs3765534 and rs11568658 of the ABCC4 gene and rs2231137 of the ABCG2 gene) and MTX plasma levels 48 hours post-infusion during the consolidation phase in 204 pediatric patients. Plasma MTX concentration levels among the different genotypes were analyzed using chi square for dichotomous discrete variables and nonparametric tests (Mann-Whitney and Kruskal-Wallis) as appropriate. The normality and homogeneity of variances of the variables were analyzed using the Shapiro-Wilk test and the Bartlett test, respectively. The results did not show a significant association between the different polymorphisms and the presence or absence of MTX toxicity in each pulse. However, a significant relationship was found between the rs2231137 (ABCG2) variant and MTX toxicity in the total pulse. Hence, the sample was divided into two, according to the dose of MTX received (2 and 5 mg/m2). In those patients who received 2 mg/m2, a significant relationship was observed between rs2231137 and the total pulse. Taking into account the patients’ variability in the MTX elimination, it would be advisable to enlarge the study sample, analyze other variants of these genes, and other genes involved in the absorption, distribution and elimination processes of MTX and in its metabolism. Moreover, another approach could be to study renal clearance as an indicator of toxicity.
