Cuantificación de los cambios morfológicos del parénquima hepático durante la ablación por microondas - Estudio ex-vivo

Resumen:
En el presente análisis experimental, se intenta caracterizar mediante tomografía computada el desplazamiento del tejido hepático bovino en un modelo ex-vivo, sometido a ablación por microondas (MO). Se propone como objetivo evidenciar el comportamiento del parénquima hepático frente a la energía térmica irradiada por la MO. Resaltando su importancia en el ámbito de la medicina ya que el hepatocarcinoma tiene una gran morbimortalidad a nivel regional y mundial, siendo causante de un porcentaje relevante de muertes por año. Su importancia no solo radica en su letalidad siendo la lesión primaria hepática de mayor frecuencia, si no la posibilidad de realizar un diagnóstico oportuno y precoz en pacientes con hepatopatía. Al incorporarse técnicas menos invasivas en el tratamiento de los tumores hepáticos, como la ablación percutánea. Esta se define exitosa cuando se logra la destrucción total del tumor hepático sin extirparlo. El objetivo de la ablación es obtener la necrosis total y su efectividad depende del tamaño, localización, vascularización tumoral y el equipamiento disponible. Estas técnicas pueden tener un fin curativo, paliativo o para realizar un “downstaging” (en hepatocarcinoma en hígados cirróticos) para que los pacientes sean candidatos posteriormente a lista de espera para trasplante hepático. Se intenta abarcar pacientes con numerosas lesiones o con criterio de irresecabilidad, dando una posibilidad, para el tratamiento curativo. Para esto se utilizó un modelo conformado por la pieza a ablacionar sobre una tabla de madera rectangular. Se utilizaron nueve piezas hepáticas que se ablacionaron por MO y se analizó mediante tomografía computada el desplazamiento de tres filamentos colocados a lo largo de la aguja de MO en función del tiempo. La antena de MO fue colocada en la convergencia de dos ejes “X” e “Y”, ambos perpendiculares entre sí. Estos ejes fueron conformados cada uno por tres filamentos. Las variables planteadas son el tiempo de ablación y la distancia de los marcadores en relación a la antena. En el modelo utilizado, se observó el desplazamiento en los ejes correspondientes al plano transversal y longitudinal, obteniéndose como resultado conductas distintas. Se observó en el eje “X” una contracción mientras que en el eje “Y”, por el contrario, se observó una dilatación. El modelo tiene algunas limitaciones y no puede ser extrapolado al modelo “in vivo” por múltiples factores. La ablación por MO es una técnica innovadora por lo que pensamos que este estudio pueda ser de utilidad para futuros análisis. Se logró demostrar la deformidad del parénquima hepático frente al proceso de ablación térmica.
In the present experimental analysis, an attempt is made to characterize by means of computed tomography the displacement of bovine liver tissue in an ex-vivo model, subjected to microwave ablation (MO). Its importance in the field of medicine since hepatocarcinoma has a high morbidity and mortality at a regional and global level, being the cause of a relevant percentage of deaths per year. Its importance lies not only in its lethality, being the most frequent primary liver lesion, but also in the possibility of making a timely and early diagnosis in patients with said liver disease. The objective is to show the behavior of the hepatic parenchyma against the thermal energy radiated by the MO. By incorporating less invasive techniques in liver tumors, such as percutaneous ablation. This is defined as successful when the total destruction of the liver tumor is achieved without removing it. The objective of ablation is to obtain total necrosis and its effectiveness depends on the size, location, tumor vascularization and the equipment available. These techniques can have a curative, palliative purpose or to perform a "downstaging" (in hepatocellular carcinoma in cirrhotic livers) so that patients are later candidates for the waiting list for liver transplantation. An attempt is made to cover patients with numerous lesions or with criteria of unresectability, giving a possibility for curative treatment.For this, a model made up of the piece to be ablated on a rectangular wooden board was used. Nine liver pieces that were ablated by MO were used and the displacement of three filaments placed along the MO needle as a function of time was analyzed by computed tomography. The MO antenna was placed at the convergence of two axes "x" and "y", both perpendicular to each other. These axes were each made up of three filaments. The variables proposed are the ablation time and the distance of the markers in relation to the antenna. In the model used, displacement was observed in the axes corresponding to the transverse and longitudinal planes, obtaining different behaviors as a result. A greater displacement (contraction) was observed on the "x" axis, while on the "y" axis, on the contrary, a dilation was observed. The model has some limitations and cannot be extrapolated to the "in vivo" model due to multiple factors. MO ablation is an innovative technique, so we think that this study may be useful for future analyses. It was possible to demonstrate the deformity of the liver parenchyma against the thermal ablation process.
