Variantes genéticas y respuesta al metotrexato en pacientes pediátricos con Leucemia Linfoblástica Aguda Laboratorio de Genética Molecular Humana, Dpto. de Ciencias Biológicas, CENUR LN, UdelaR, Salto 2022

Resumen:

Objetivo: Analizar el impacto de variantes en los genes MTHFR, TYMS, SLC19A1 y SLCO1B1 en la respuesta hematológica al metotrexato en pacientes pediátricos con Leucemia Linfoblástica Aguda. Metodología: Estudio observacional de cohorte retrospectivo. Se utilizaron datos de 137 pacientes con diagnóstico de LLA, tratados con el protocolo BFM en el Servicio Hemato-Oncológico Pediátrico del Centro Hospitalario Pereira Rossell, obtenidos del proyecto ANII FMV_3_2018_148458 cuya responsable es la Dra. Ana María Soler. Se registraron las siguientes variables: valores de leucocitos y neutrófilos pre y post infusión de metotrexato. Se analizó si cada una de las variantes génicas por separado influye en el cambio del número de leucocitos y neutrófilos luego de cada pulso de metotrexato. Resultados: Los polimorfismos analizados en los genes MTHFR, TYMS, SLCO1B1 no mostraron una asociación estadísticamente significativa, ni con el cambio en el número de glóbulos blancos ni con el porcentaje de neutrófilos pre y post administración de metotrexato, para ninguno de los cuatro pulsos de la fase de consolidación. Sin embargo, se encontró una asociación estadísticamente significativa entre el polimorfismo rs1051266 del gen SLC19A1 y el cambio de número de glóbulos blancos luego de la administración de metotrexato en el 2do pulso de la fase de consolidación. Conclusiones: Los resultados obtenidos no son concluyentes para definir que las variantes estudiadas influyen en la respuesta al metotrexato en la reducción de glóbulos blancos pre y post pulso. Sin embargo, el polimorfismo rs1051266 del gen SLC19A1 fue estadísticamente significativo en el pulso 2 y 3 post metotrexato.

Objective: To analyze the impact of MTHFR, TYMS, SLC19A1 and SLCO1B1 gene variants on the hematological response to Methotrexate in Acute Lymphoblastic Leukemia pediatric patients. Methodology: Retrospective observational cohort study. Data from 137 patients diagnosed with ALL, treated with the BFM protocol at Servicio Hemato Oncológico Pediátrico del Centro Hospitalario Pereira Rossell, obtained from the ANII FMV_3_2018_148458 project whose manager is Dr. Ana María Soler, were used. The following variables were recorded: pre and post methotrexate infusion leukocyte and neutrophil values. It was analyzed whether each of the genetic variants influences the number of leukocytes and neutrophils after each methotrexate pulse. Results: The polymorphisms analyzed in MTHFR, TYMS, and SLCO1B1 genes did not show a statistically significant association neither with the change in the number of white blood cells nor with neutrophils percentage before and after methotrexate administration in any of the four courses of consolidation phase. However, a statistically significant association was found between the rs1051266 of the SLC19A1 gene and the change in the number of white blood cells after methotrexate administration in the consolidation phase 2nd course. Conclusions: The results obtained are not conclusive to define that the variants studied influence the response to methotrexate in the reduction of white blood cells pre and post pulse. However, the rs1051266 polymorphism of the SLC19A1 gene was statistically significant at pulse 2 and 3 post methotrexate.
