Resumen:

Introducción: La arteria de Percherón es una variante anatómica que se encarga de la irrigación bilateral de ambos tálamos mediales y sector rostral del tronco encefálico. Su obstrucción determina infartos en dichos territorios. Objetivo: Describir un caso clínico de infarto de la arteria de Percherón destacando los hallazgos clínicos e imagenológicos, para considerar esta patología en población pediátrica con compromiso de conciencia agudo. Caso Clínico: Adolescente de 17 años sana que presentó un cuadro de compromiso de conciencia y diplopía, la cual revierte en pocas horas. Me diante resonancia magnética de encéfalo se objetivó un infarto talámico medial bilateral, diagnos ticándose una oclusión de la arteria de Percherón. Como único elemento presuntamente etiológico se objetivó la presencia de un foramen oval permeable. Se realizó corrección quirúrgica del defecto cardiaco y anticoagulación, con recuperación completa del cuadro. Conclusión: Es fundamental conocer el patrón clínico-radiológico de este cuadro, el cual es muy característico, pero de baja frecuencia en la edad pediátrica.

Introduction: Artery of Percheron (AOP) is an anatomical variant responsible for the bilateral irrigation of both the medial thalami and rostral sector of the brainstem. Its obstruction causes infarcts in these areas. Objective: To describe a clinical case of AOP infarction, highlighting the clinical and imaging fin dings to consider this pathology in the pediatric population with acute altered state of consciousness. Clinical case: A healthy 17-year-old adolescent presented with altered state of consciousness and diplopia, which was resolved in a few hours. Brain MRI showed a bilateral medial thalamic infarction, diagnosing an occlusion of the AOP. The only presumed etiological element was the presence of a patent foramen ovale. Surgical correction of the cardiac defect and anticoagulation were performed, with complete recovery. Conclusion: It is essential to know the clinical-radiological pattern of this condition, which is very characteristic, but infrequent in the pediatric age.
