Valoración de la respuesta humoral a la vacunación contra SARS-Cov-2 en una muestra de pacientes con nefropatía lúpica

Introducción: La mejor medida de control lograda para la pandemia por SARS-CoV-2 ha sido el desarrollo de vacunas. Los pacientes con Nefropatía Lúpica (NL) no han sido incluidos en estudios que comprueben la eficacia de las mismas en dicha población. Objetivo: Analizar la capacidad de generar respuesta inmunológica humoral frente a la vacunación contra SARS-CoV-2, en pacientes con NL bajo tratamiento inmunosupresor, seguidos en Policlínica de Nefrología en el Hospital de Clínicas, Montevideo, Uruguay. Métodos: Se realizó un estudio transversal, en pacientes con NL a quienes se extrajo sangre periférica al mes de la segunda vacuna para SARS-CoV-2. Se buscó la presencia de anticuerpos (AC) por test de ELISA, sensibilizado con el fragmento Receptor Binding Domain (RBD) de la proteína Spike del SARS-CoV-2. También se registraron efectos adversos y variaciones en el score SLEDAI luego de administrada la vacuna. Resultados: Del total de pacientes incluidos (19), la mediana de edad fue de 35,5 años, siendo un 89% correspondiente al sexo femenino. La tasa global de seroconversión fue de 63% (p=0,11). Los que estaban en remisión completa (RC) generaron una respuesta de Ac significativamente mayor. Los vacunados con Pfizer generaron anticuerpos en un 83% (p=0.03), mientras que los vacunados con Sinovac, lo hicieron en un 54% (p=0.69). Aquellos tratados inicialmente con Ciclofosfamida, presentaron una diferencia significativa a favor de la generación de anticuerpos (p=0.009). Los pacientes en tratamiento actual con Micofenolato presentaron resultados positivos en un 58% de los casos (p=0.33); con Prednisona el 55% (p=0.65); y con Hidroxicloroquina el 61% (p=0.19). No se modifico la actividad de la enfermedad de base. El efecto adverso más prevalente fue dolor (52%). Conclusiones: Se logra mayor seroconversión anti-SARS-CoV-2 en pacientes en remisión completa o vacunados con Pfizer. No existieron efectos adversos graves ni variaciones en el score SLEDAI luego de la vacunación.
Introduction: The most effective strategy to control SARS-CoV-2 pandemic has been vaccination. Patients with Lupus Nephritis (NL) have not been included in any research that studies vaccine efficacy in this population. Objective: Analyse the ability to generate a humoral immune response to vaccination against SARS-CoV-2 in patients with Lupus Nephritis under immunosuppressive treatment, followed by the Nephrology Polyclinic at Hospital de Clínicas, Montevideo, Uruguay. Methods: A cross-sectional study was carried out in patients with NL from whom peripheral blood was drawn one month after the second SARS-CoV-2 vaccine. The presence of antibodies was quantified by ELISA test, sensitized by the Receptor Binding Domain (RBD) fragment of the Spike protein of SARS-CoV-2. Adverse effects and variations in the SLEDAI score were also documented after the vaccine was administered. Results: From the total number of patients included (19), the median age was 35.5 years, with 89% corresponding to the female sex. The overall seroconversion rate was 63% (p = 0.11). Those in complete remission (CR) generated a significantly higher antibody response. 83% of patients vaccinated with Pfizer generated antibodies (p = 0.03), while those vaccinated with Sinovac, only 54% did so (p = 0.69). Those initially treated with Cyclophosphamide showed a significant difference in favor of the generation of antibodies (p = 0.009). The patients in current treatment with Mycophenolate presented positive results in 58% of the cases (p = 0.33); with Prednisone 55% (p = 0.65); and with Hydroxychloroquine 61% (p = 0.19). The activity of the underlying disease was not modified. The most prevalent adverse effect was pain (52%). Conclusions: Greater anti-SARS-CoV-2 seroconversion is achieved in patients vaccinated with Pfizer or in complete remission. There were no serious adverse effects or variations in the SLEDAI score after vaccination.
