Resumen:
Objetivo: Identificar variables fisiológicas asociadas a la realización de prueba de ventilación espontánea en pacientes con soporte ventilatorio mecánico. Materiales y métodos: Se realizará un estudio descriptivo observacional transversal, donde analizaremos los casos clínicos de 6 pacientes neurocríticos ingresados en el Centro de Terapia Intensiva (CTI) del Hospital de Clínicas (HC), que se encuentren en condiciones de realizar la prueba de ventilación espontánea, a forma de ensayo piloto. Se registrarán y analizarán 3 tipos de variables mediante un sistema de cámaras de video: signos vitales, variables del equipo de Asistencia Ventilatoria Mecánica (AVM) y variables de capnografía. Resultados: Todas las PVE fueron bien toleradas y representaron un esfuerzo cardiopulmonar moderado. En 5 de 6 casos se identificó una disminución de la ventilación alveolar a lo largo de la prueba. Se identificaron al menos 2 mecanismos probablemente asociados a esta reducción de la ventilación alveolar: fatiga muscular y asincronía paciente-ventilador. Conclusiones: Es necesario ampliar el número de pacientes para poder determinar si existen patrones de comportamiento diferentes en pacientes neurocríticos comparándolos con el resto de los pacientes graves.
Objective: To identify physiological variables associated with the performance of spontaneous ventilation test in patients with mechanical ventilatory support. Materials and methods: A cross-sectional observational descriptive study will be carried out, where we will analyze the clinical cases 6 neurocritical patients admitted to the Intensive Care Center (ICU) of the Hospital de Clínicas (HC), who are able to perform the spontaneous ventilation test, as a pilot test. Three types of variables will be recorded and analyzed by a video camera system: vital signs, equipment variables of Mechanical Ventilatory Assistance (AVM) and capnography variables. Results: All PVEs were well tolerated and represented moderate cardiopulmonary effort. In 5 of 6 cases, a decrease in alveolar ventilation was identified throughout the test. At least 2 mechanisms probably associated with this reduction in alveolar ventilation were identified: muscle fatigue and patient-ventilator asynchrony. Conclusions: It is necessary to expand the number of patients to be able to determine if there are different patterns of behavior in neurocritical patients compared to the rest of the serious patients.
