Evaluación del riesgo cardiovascular en la artritis reumatoide

Resumen:

La artritis reumatoide está asociada con un aumento en la morbimortalidad cardiovascular e incluso constituye un factor de riesgo cardiovascular independiente. Objetivo: Realizar una puesta al día sobre la evaluación del riesgo cardiovascular en pacientes con artritis reumatoide. Valorar los diferentes scores de estimación de riesgo cardiovascular, describir la utilidad de la ultrasonografía carotídea en la re-estratificación del mismo y evaluar el efecto del tratamiento farmacológico de la artritis reumatoide sobre el riesgo cardiovascular. Metodología: Se realizó una búsqueda bibliográfica en distintas bases de datos, seleccionando los artículos más recientes que se consideraron pertinentes a los objetivos planteados, sin hacer restricciones al tipo de estudio. Resultados: Hasta el momento ningún score ha podido determinar el riesgo cardiovascular real en los pacientes con artritis reumatoide. Este riesgo no se explica exclusivamente por los factores tradicionales, sino que también intervienen factores de riesgo específicos, como la actividad inflamatoria de la enfermedad, que contribuye a la aterosclerosis acelerada. Se destaca el empleo de la ultrasonografía carotídea en la re-estratificación del riesgo cardiovascular en artritis reumatoide, especialmente en pacientes con riesgo cardiovascular intermedio. El tratamiento precoz de la artritis reumatoide con fármacos modificadores de la enfermedad, siguiendo la estrategia “treat-to-target”, pretende controlar la actividad de la enfermedad y así disminuir el riesgo cardiovascular. Conclusiones: Los estudios sobre la evaluación de riesgo cardiovascular en artritis reumatoide no son concluyentes, y no hay consenso sobre el manejo clínico cardiovascular de pacientes con esta enfermedad. Sería beneficioso fortalecer el trabajo interdisciplinario entre reumatólogos y cardiólogos para una valoración más integral de la enfermedad.

Rheumatoid arthritis is associated with an increase in cardiovascular morbidity and mortality and even represents an independent cardiovascular risk factor itself. Aim: Assess the present literature on the evaluation of cardiovascular risk in patients with rheumatoid arthritis and the different cardiovascular risk estimation scores. Describe the importance of carotid ultrasonography in the re-stratification of this risk and the effect of thepharmacological treatment of rheumatoid arthritis on the cardiovascular risk. Methods: A bibliographic search was carried out in different databases, selecting the most recent articles that were considered relevant to the proposed objectives, without restricting the type of study. Results: So far no score has been able to assess the real cardiovascular risk in patients with rheumatoid arthritis. This risk is not fully explained by traditional risk factors, it also involves specific factors, such as the inflammatory activity of the disease, which contributes to accelerated atherosclerosis. The use of carotid ultrasonography in the re-stratification of cardiovascular risk in rheumatoid arthritis is highlighted, especially in patients with intermediate cardiovascular risk. The early treatment of rheumatoid arthritis with disease-modifying drugs, following the “treat-to-target” strategy, aims to control the activity of the disease and thus reduce cardiovascular risk. Conclusions: The studies on the evaluation of cardiovascular risk in rheumatoid arthritis are inconclusive, there is no consensus on the clinical cardiovascular management of patients with this disease. It would be beneficial to strengthen interdisciplinary work between rheumatologists and cardiologists for a more comprehensive assessment of the disease.
