Validación de un score pronóstico de enfermedad renal crónica: KFRE (Kidney Failure Risk Equation) de ocho variables. Hospital de Clínicas “Doctor Manuel Quintela”, Programa de Salud Renal del Uruguay, 2004-2020

Resumen:

KFRE predice el riesgo de ingreso a terapia de sustitución renal (TRS) a 2 o 5 años en pacientes con enfermedad renal crónica (ERC). Incluye 4 u 8 variables asociadas al pronóstico renal. La fórmula de 8 variables aún no ha sido validada en Uruguay ni en América Latina. El Programa Nacional de Salud Renal de Uruguay (PSRU) cubre 80% de la población. Los pacientes ingresan al Registro Nacional de ERC con datos iniciales y evolutivos. Objetivo: validar KFRE de 8 variables para su aplicación a la población uruguaya portadora de ERC. Metodología: estudio observacional, retrospectivo, longitudinal, de cohorte. Se incluyeron pacientes del PSRU ingresados entre 2004-2020 en los cuales se realizó seguimiento por al menos 5 años. KFRE de 8 variables incluye: edad, sexo, tasa de filtrado glomerular, índice albuminuria/creatininuria (ACR), bicarbonatemia, calcemia, fosfatemia y PTH. Para evaluar la calibración y capacidad de discriminación se analizó el área bajo la curva ROC (AUC) por estratos y la relación de eventos estimados y observados, y se comparó con KFRE de 4 variables. El Proyecto fue aprobado por Comité de Ética. Se utilizó software IBM-SPSS v29. Resultados: se incluyeron 668 pacientes. KFRE de 8 variables mostró AUC de 0,903 a 2 años y de 0,898 a 5 años. La frecuencia de eventos esperados (20,2%) y observados (23,2%) fue similar. En el quintilo de mayor riesgo (probabilidad de TRS>38%) la tasa de eventos esperados (74,5%) y observados (71,9%) fue similar (p=NS), con mejor ajuste que KFRE 4 variables en pacientes de alto riesgo. Conclusiones: KFRE estima adecuadamente el riesgo de requerimiento de TRS en todos los estratos de riesgo.

KFRE accurately predicts the risk of admission to renal replacement therapy (RRT) for patients with chronic kidney disease (CKD) at 2 or 5 years. It utilizes four or eight variables associated with renal prognosis, though the latter has yet to be validated in Uruguay or Latin America. The National Renal Health Program of Uruguay (PSRU) covers 80% of the population and patients are enrolled in the National CKD Registry, where initial and evolving data is recorded. Objective: To validate KFRE of eight variables for its application to the Uruguayan population carrying CKD. Methods: this is an observational, retrospective, longitudinal, cohort study. The patients admitted to PSRU between 2004-2020 who were followed up for at least 5 years were included. KFRE of eight variables includes age, gender, glomerular filtration rate, albuminuria/creatininuria ratio (ACR), bicarbonate, calcium, phosphate and PTH. To assess calibration and discrimination capacity, the area under the ROC curve (AUC) was analysed by strata, as well as the comparison of estimated and observed events with KFRE of 4 variables. The project received approval from the Ethics Committee and IBM-SPSS v29 software was used. Results: 668 patients were included, and the AUC for KFRE with 8 variables was 0.903 at 2 years and 0.898 at 5 years. The frequency of expected (20.2%) and observed (23.2%) events was similar. In the highest-risk quintile (probability of RRT >38%), there was no significant difference between the expected (74.5%) and observed (71.9%) event rates (p=NS), with better fit than KFRE 4 variables in high-risk patients. Conclusion: KFRE appropriately estimates the risk of RRT requirement in all risk strata.
