Breast cancer (BC) has the highest incidence and morbidity-mortality rates in women in Uruguay and worldwide. The overexpression of the HER2 oncogene, present in 15-25% of breast cancers, is associated with a worse prognosis, partially reversed through the incorporation of anti-HER2 targeted therapies. As the first-line treatment in advanced stages, trastuzumab, in association with chemotherapy, was the first drug to demonstrate an improvement in overall survival (OS). Subsequently, the combination with pertuzumab showed even better results, currently being the standard of treatment. Additionally, multiple HER2-targeted treatments for second and subsequent lines have revolutionized therapy. In our country, there is universal coverage for trastuzumab, pertuzumab, and TDM1 through the National Resources Fund (FNR). Given the crucial importance of proper financial resource management for low-middle-income countries, we find it particularly interesting to study the characteristics and evolution of patients treated with trastuzumab/pertuzumab (standard treatment) according to FNR regulations, aiming to provide useful data for medical professionals, patients, and policymakers involved in financing. Objective: Evaluate the efficacy of anti-HER2 treatment with first-line trastuzumab plus pertuzumab plus chemotherapy in Uruguayan patients with advanced HER2-positive BC, financed by the FNR, from January 2008 to July 2023. Methodology: A retrospective observational study was conducted using anonymized FNR data from patients with advanced stage HER2-positive BC from January 2008 to July 2023. Conclusion: With national evidence it was concluded that trastuzumab associated with pertuzumab and chemotherapy is still being the treatment with biggest efficacy to treat BC HER2+.
