Hepatotoxicidad por fármacos antituberculosos: ¿qué factores se ven implicados en su desarrollo?

Resumen:

Introducción: La tuberculosis se considera un problema de salud pública cuyo tratamiento consta de la combinación de cuatro antibióticos: rifampicina, pirazinamida, isoniacida y etambutol. La hepatotoxicidad es reconocida como una de las principales complicaciones del mismo, lo que contribuye al fracaso y no adherencia al tratamiento, favoreciendo la aparición de farmacorresistencia. Objetivo: Conocer los factores que favorecen el desarrollo de hepatotoxicidad inducida por fármacos antituberculosos. Metodología: Se realiza una búsqueda bibliográfica de artículos publicados a partir del 2010, utilizando las bases de datos PUBMED, Portal Timbó y Scielo principalmente. Resultados: Se hallaron 963 artículos, de los cuales se seleccionaron 238 y 44 fueron incluidos en esta revisión. Los factores de riesgo que se tomaron en cuenta fueron: sexo, edad, malnutrición, infección por virus de hepatitis B y C, VIH, y polimorfismo genético. Este último es el que aparece como el factor más prometedor al momento de determinar el desarrollo de hepatotoxicidad para estos pacientes bajo tratamiento. Conclusiones: Los hallazgos podrían contribuir a lograr ajustar la dosis de los fármacos antituberculosos para cada paciente de acuerdo a los factores de riesgo que presente, evitando así el desarrollo de efectos adversos a nivel hepático.

Introduction: Tuberculosis is considered a public health problem whose treatment consists of the combination of four antibiotics: rifampicin, pyrazinamide, isoniazid and ethambutol. The hepatotoxicity is recognized as one of the main complications of it, which contributes to failure and non-adherence to treatment, favoring the appearance of drug resistance. Objective: To know the factors that favor the development of hepatotoxicity induced by antituberculous drugs. Methodology: A bibliographic search was carried out of articles published since 2010, using the PUBMED database, Portal Timbó and Scielo mainly. Results: 963 articles were found, of which 238 were selected and 44 were included in this review. The risk factors that were taken into account were: sex, age, malnutrition, hepatitis B and C virus infection, HIV, and genetic polymorphism. The latter is the one that appears as the most promising factor when determining the development of hepatotoxicity for these patients under treatment. Conclusions: The findings could help to adjust the dose of anti-tuberculosis drugs for each patient of according to the risk factors present, thus avoiding the development of adverse effects at the hepatic.
