Resumen:

Introducción: El cáncer de riñón metastásico continúa siendo al momento actual un desafío terapéutico para la uro-oncología. Con el advenimiento de las nuevas terapias sistémicas, se vuelve cada vez más discutida la indicación de la nefrectomía citorreductora de inicio, siendo aún controversial el tratamiento quirúrgico sobre el tumor primario en un estadio con compromiso sistémico. Materiales y Métodos: Mediante la realización de una encuesta a centros hospitalarios en Latinoamérica, esta publicación pretende evaluar la indicación de la nefrectomía citorreductora, la técnica quirúrgica empleada y resultados obtenidos, estimando la predicción de líneas terapéuticas regionales. El equipo de investigación diseño una encuesta en formato cuestionario online, basado en los principales puntos de discusión según la bibliografía internacional sobre la nefrectomía citorreductora y el cáncer renal metastásico. Resultados: Un total de 20 centros respondieron de forma completa la pesquisa, afirmativamente con respecto a la realización de nefrectomía citorreductora. La mayoría ofrecen además tratamiento sistémico. En cuanto a la técnica quirúrgica empleada, la mayoría utilizaban técnicas mínimamente invasivas. Discusión: La evidencia internacional con respecto al rol de la nefrectomía citoreductora en este contexto es variada y sujeta a permanente cambio. El tratamiento individualizado y en marco de equipo multidisciplinario fundamental para el abordaje de estos pacientes. Conclusiones: El subgrupo de pacientes con cáncer de riñón metastásicos es conceptualmente heterogéneo. La nefrectomía citoreductora sigue jugando un rol importante, a veces protagónico en el manejo de algunos pacientes seleccionados.

Introduction: Metastatic kidney cancer continues to be a therapeutic challenge for urologies and oncologies. With the new systemic therapies, the indication for initial cytoreductive nephrectomy is becoming increasingly controversial. The treatment of the primary tumor at a stage with systemic involvement continues to be a controversial issue. Materials and Methods: By conducting a survey of Latin American hospitals, this publication aims to evaluate the indication for cytoreductive nephrectomy, the preferred surgical technique and the results obtained, estimating regional therapeutic lines. Online survey was made, based on the most important topics of discussion on cytoreductive nephrectomy and metastatic kidney cancer. Results: A total of 20 centers participated on the survey, al of them affirmatively about the realization of cytoreductive nephrectomy. Most of them also offer systemic treatment. Regarding the surgical technique used, most used minimally invasive techniques. Discussion: The international evidence about the role of cytoreductive nephrectomy in this context is varied and subject to permanent change. Individualized treatment, multidisciplinary team are fundamental for the approach of these patients. Conclusions: The subgroup of patients with metastatic kidney cancer is conceptually heterogeneous. Cytoreductive nephrectomy continues to play an important role, sometimes leading role, in the management of selected patients.
