Introduction: Kidney transplantation (KT) is the treatment of choice in end-stage renal disease. Barriers to timely access to KT are linked to the patient and the health system. Objective: To know the time of the pre-transplant evaluation process of the Hospital de Clínicas Center and identify possible delay factors. Methodology: Retrospective, observational cohort study. We reviewed medical records and included patients in the pre-transplant evaluation of the Hospital de Clínicas between 01-01-2018 and 07-31-2021. Two groups were defined according to delays in the study process: with and without drawbacks. Results: Most of the patients (83.4%) were included in the group with problems in the evaluation process, with a more extended study time than those without problems (eleven vs. four months, p = 0.000). The most frequent cause of delay was linked to patient factors (64%). The most frequent temporary contraindication was obesity (54%). Patients with a public provider compared to a private provider had a higher proportion of inconveniences (86.7% vs. 75.9%, p <0.05), and the reference time to pre-transplantation concerning the start of dialysis was longer (median eleven vs. five months, p <0.05). After 2020, the number of first consultations decreased (without significant differences), and the number of patients with problems decreased (51% vs. 86%, p = 0.000). Conclusions: Most patients presented inconveniences during the pre-transplant evaluation process that delayed their admission to the waiting list. The main inconvenience detected was the patient with non-attendance at consultations and/or difficulties in coordinating studies. In the future, it is essential to design strategies that take into account these difficulties and evaluate psychosocial factors and management of the health system.
