Calidad de vida relacionada con la salud en pacientes atendidos en el Centro de Esclerosis Lateral Amiotrófica del Uruguay en 2023

La Esclerosis Lateral Amiotrófica (ELA) es una enfermedad neurodegenerativa progresiva que afecta las motoneuronas superiores e inferiores, ocasionando manifestaciones clínicas variables y heterogéneas que afectan la funcionalidad y calidad de vida de los pacientes. Al momento, no se cuentan con datos de la población uruguaya sobre la calidad de vida de los pacientes diagnosticados con ELA. Esta investigación buscó dar a conocer la Calidad de Vida Relacionada con la Salud en pacientes diagnosticados con ELA que concurrieron al Centro de Esclerosis Lateral Amiotrófica del Uruguay (CELAU) del Hospital de Clínicas Dr. Manuel Quintela durante el período de julio a finales de octubre del año 2023. Se llevó a cabo un estudio observacional de tipo transversal descriptivo, donde se utilizó la escala EQ-5D-5L como medida de salud autopercibida, un cuestionario ad hoc para los datos sociodemográficos relevantes, la Escala de Ansiedad y Depresión Hospitalaria (HADS) con el fin de evaluar la prevalencia de ansiedad y depresión. Se utilizó además la escala ALSFRS-R que fue aplicada por neurólogos del CELAU. De una muestra de 23 pacientes, el 73,9% presentaron inicio espinal, mientras que el 26,1% fue de inicio bulbar. Se observó una relación significativa entre el EQ-5D-5L y el ALSFRS-R (p<0.01), así como una correlación negativa moderada entre los datos del EQ-5D-5L y las subescalas del HADS-A/D (p=0,03/p<0.01). La prevalencia de depresión fue de 43,4% y para la ansiedad de 60,8%. Existe una fuerte correlación entre el EQ-VAS y el índice general del EQ-5D-5L. La declinación funcional y la depresión son buenos predictores de la calidad de vida de esta población. El EQ-VAS es una buena medida de calidad de vida autopercibida.
Amyotrophic Lateral Sclerosis (ALS) is a progressive neurodegenerative disease that affects upper and lower motor neurons, causing variable and heterogeneous clinical manifestations that impact the functionality and quality of life of patients. Currently, there is a lack of data for the Uruguayan population regarding the quality of life of individuals diagnosed with ALS. This research aimed to appraise the health-related quality of life in patients diagnosed with ALS who attended the Centro de Esclerosis Lateral Amiotrófica del Uruguay (CELAU) of the Hospital de Clínicas, Dr. Manuel Quintela, from July to the end of October 2023. A descriptive cross-sectional observational study was conducted, where the EQ-5D-5L scale was used as a measure of self-perceived health, an ad hoc questionnaire for relevant sociodemographic data, the Hospital Anxiety and Depression Scale (HADS) to evaluate the prevalence of anxiety and depression. In addition, the result of the ALSFRS-R scale was obtained, which was applied by CELAU’s neurologists. Among a sample of 23 patients, 73.9% had spinal onset, while 26.1% showed bulbar onset. A significant association was observed between the EQ-5D-5L and the ALSFRS-R (p<0.01), as well as a moderate negative correlation between the EQ-5D-5L data and the HADS-A/D subscales (p= 0.03/p<0.01). The prevalence of depression was 43.4% whereas for anxiety was 60.8%. The coefficients of determination between all scales were studied, obtaining them for the HADS-D and ALSFRS-R scores. There is a strong correlation between the EQ-VAS and the general index of the EQ-5D-5L. Functional decline and depression are good predictors of quality of life in this population. The EQ-VAS is a good measure of self-perceived quality of life.
