Resumen:

After childbirth a patient underwent surgery for choleperitoneum due to permeation. During pregnancy she had suffered from obstructive jaundice. Surgerywas performed in an acute phase, 15 litres of biliary liquid were drained from a preperitoneal pouch but the biliary tract, which was not drained, was not perforated.A fortnight later cholelithiasis was suspected, she was re-oper-ated _and a gallstone imbeded in the papilla was removed; this was the determinant cause of both jaundice and choleperitoneum and had possibly been accompanied by acute pancreatitis and backflow caused by Opie's mechanism.


Relatamos una observación de peritonitis biliar por permeación operada después del parto. La pacientedurante el embarazo había hecho una ictericia obstructiva. Se operó en agudo evacuando 15 litros de líquido bilioso de una bolsa preperitoneal sin encontrarperforación de la vía biliar que no se drenó. Con la sospecha de litiasis coledociana fue reintervenida 15 días después, -extrayéndosele un cálculo enclavadoen la papila que había determinado secundariamente la ictericia y la peritonitis biliar; acompañada posiblemente de pancreatitis aguda y reflujo por el mecanismo de Opie.
