Direct surgery of the deep femoral artery in chronic obstructive arteriopathies of the lower limbs

Cirugía directa de la arteria femoral profunda en las arteriopatías obstructivas crónicas de los miembros inferiores

Resumen:

Analysis of an uncommon therapeutic approach in arterial occlusive diseases of lower limbs: surgery limited to the profunda femoris artery. Eleven procedures were performed on 10 patients: 9 endarterectomies with vein patches and 2 pure angioplastiae with vein patches. In 6 cases endarterectomy and complementary vein patch were performed in the initial segment of the superficial femoral artery, in order to preserve its patency. Results were satisfactory in 9 cases, with clear improvement of the ischemia symptoms. The anatomical, surgical, pathological and physiological basis of this procedure are discussed. The indications of this technique derive from the impossibi!ity of using the conventional methods to restare circulation (by pass grafts or direct disobstruction of the arterial axis). From the different technical variants described, endarterectomy and closing with a vein patch is preferred.


Análisis de un planteo terapéutico no habitual de las arteriopatías obstructivas de los miembros inferiores: la cirugía limitada a la arteria femoral profunda. En 10 enfermos se realizaron 11 procedimientos operatorios: 9 endarterectomías con parche de vena y 2 angioplastias puras con parche venoso. En 6 casos se efectuó endarterectomía y parche venoso complementario en el sector inicial de la femoral superficial, para conservar supermeabilidad. Los resultados fueron buenos en 9 casos, con mejoría neta de los síntumas isquémicos. Se estudian los fundamentos anatomoquirúrgicos, anatomopatológicos y fisiopatológicos de este procedimiento, cuyas indicaciones derivan de la imposibilidacl de utilización ele los métodos clásicos de revascularización (by-pass o  desobstrucción directa del eje arterial). De las distintas variantes técnicas que se describen, se prefiere la endarterectomía y cierre con parche venoso.
