Peritonitis de origen pielorrenal
Resumen:
Five cases of peritonitis of pyelorenal origin are reported upon. Four of them were of the spontaneous type undergoing unspecific pyelonephritis and pyonephrosis,consistent a;ssociated with pyelocalicial dilation and, in two instances, with lithiasis. A discussion is carried out of the etiologic and pathologic fe,atures, as well as ofthose related with the clinical and operative diagnosis and with emergency therapy. Stress is laid on the fact that positive diagnosis is essential in the adequate planningof surgical treatment.
Se presentan cinco observaciones de peritonitis de origen pielorrenal. Cuatro de ellas son de tipo espontáneo en procesos de pielonefritis y pionefrosis inespecíficas,asociadas regularmente a la dilatación pielocalicial y en dos casos a la litiasis. La observación restante es una forma postoperatoria luego de una pielonefrostomía en una pionefrosis litiásica. Se discuten los aspectos etiológicos y patológicos, así como los relativos al diagnóstico clínico y operatorio y a las medidas terapéuticas de urgencia.Se insiste en que el diagnóstico es fundamental para encaminar el tratamiento quirúrgico en forma adecuada. Y que no se logra realizarlo si no se tiene conocimiento de su posibilidad y se piensa en ella .
