Colecistectomía laparoscópica. Experiencia con 504 casos
Resumen:
Between February 1991 and Ju/y 1993, 504 laparoscopiccholecystectomies were carried out in patients with gal/bladder and choledochal lithiasis, and acule cholecystitis.Ages ranged between 8 and 85 years; with a media of48; 218 males and 286 fema/es. There was a O, 16mortality. (One patient died 6 hours after the procedurewith a heart pacemaker.)Whenever cholecochal lithiasis was suspected, CRE wascarried out, adn laparoscopic cholecystectomy wasperformed 48 hours later. In 8 cases conventionaltechnique had to be adopted due to difficulties in hilumdisection, inflammatory sclerosis and cholecystoduodenalfistula.No reoperations were needed. There was no legion ofthe biliar vine. As our experience grew, relativecounterindications decreased; only those absolute onesfar any surgery remain.
En el período comprendido entre febrero de 1991 y juliode 1993, se efectuaron 504 colecistectomíaslaparoscópicas en pacientes con litiasis vesicular,coledociana y colecistitis aguda. Los rangos de edadoscilaron entre 8 y 85 años, con una media de 48 años,de los cuales 218 eran de sexo masculino y 286 de sexofemenino, con una mortalidad de O, 16% (un pacientefallecido 6 horas posteriores con marcapasos cardíaco).En caso de sospecha clínica de litiasis coledociana, seefectuó C.R.E. y 48 hs. posteriores se realiza lacolecistectomía laparoscópica. En 8 oportunidadesdebió convertirse a técnica convencional por dificultadesen la disección de hilio, esclerosis inflamatoria y fístulacolecistoduodenal.En ningún caso fue necesario reintervenir. No hubolesión de la vía biliar.A medida que fue creciendo nuestra experiencia, lascontraindicaciones relativas fueron disminuyendo,quedando solamente las absolutas para cualquier cirugía
