Exceptional resources in limb revascularization: Compound aortopopliteal and axilopopliteal bypass

Resumen:
The authors present 4 cases of complex revascuiarization in patients previously operated for obstructive arteriopathy of lower extremities. The belong to a series of 800 patients operated of which 304 correspond to proximal revascularization.Difficulties on the dissection of the femoral tripod owing to the first operation made it necessary to use extended bypasses with combined prothesis of dacron and vein. The utility of these procedures is discussed.Although exceptional they seem fundamental when the indication is present. The 4 cases were performed succesfully. They are still permeable at the time of this presentation with a period variable between 2 and 20 months.
Los autores presentan 4 casos de revascularización compleja en pacientes previamente operados por arteriopatía obstructiva de miembros inferiores que corresponden a una serie de 800 enfermos intervenidos, de los cuales 304 corresponden a revascularizaciones, proximales.Las dificultades de abordaje del trípode femoral debidas a la intervención anterior hicieron necesaria la utilización de puentes extendidos con prótesis de dacrón y venas combinados.Se discute la utilidad de estos procedhnientos, que aunque excepcionales son importantes cuando la situación indicada se presenta.Los 4 casos, se resolvieron con éxito, estando permeables en el momento de esta presentación, con un periodo variable entre 2 y 20 meses.
