Evolution away from with the procedure jejunal mount by Hivet-Warren
Evolución alejada del con el procedimiento montaje yeyunal de Hivet-Warren
Resumen:
This experiment was carried out with 33 patients who underwent high biliojejunal anastomosis with theHivet-Warren loop. Twenty-eight of these patients were suffering from obstructive cancers in the biliary trae! and five had stenosis or benign fistulas. None of themshowed clinical symptomatdogy that could be ascribed to complications derived from this technique. However in three cases, who had to be reoperated five monthslater for differents reasons the afferent branches which had been ligated were found to have been repermeabilized. In another patient, a large asymptomaticblind pouch had been formed above the stricture the paper analyzes the advantages, inconvinients and indications of the Hivet-Warren loop and the technicalvariations for preventing complications.
Se presenta la experiencia realizada en 33 pacientes a los que se les efectuaron anastomosis bilioyeyunalesaltas con asa de Hivet Warren. De ellos 28 tenían cánceres obstructivos de las vías biliares y 5 estenosis o fístulas benignas. Ninguno tuvo sintomatologia clínica imputable a complicaciones de esa técnica. Pero en 3 casos que por diversas circunstancias fueron reintervenidos después de 5 meses, se encontraronrepermeabilizadas las ramas aferentes que habían sido ligadas. En otro se había formado un voluminoso saco ciego asintomático por encima de la estrictura. Se analizan las ventajas, inconvenientes e indicaciones del asa de Hivet-Warren, y las variantes técnicas para impedir sus complicaciones.
