Resumen:

Objective: To study a series of patients with thyroid pathologies and they treatment Material and methods: Our series includes 128 patients and 132 surgeries, with 4 reoperations. The average age was 55 years; 87 women and 41 men. 105 total thyroidectomies were performed (80%). 60 cases (45%) of goiter, 41 (31%) of thyroid nodules with possible malignancy and 4 (3%) reoperations to complete a total thyroidectomy. In 27 patients (20%) a lobectomy and isthmectomy was performed Results: In the 60 operated goiters 40 involution hyperplastic nodules (67%), 12 (20%) Hashimoto´s thyroiditis and 8 (13%) diffuse parenchymatous goiters were found. In 6 cases (10%) papillary microcarcinomas were found. In 41 total thyroidectomies by thyroid nodules were found: 17 follicular adenomas (41%), 12 papillary carcinomas (29%), 7 follicular carcinomas (17%), 1 medullary carcinoma (2.4%) and other injuries. In 27 cases of lobectomy and a isthmectomy were founded: 11 hyperplastic nodules (41%), 10 follicular adenomas (37%), 3 follicular carcinomas (11%) and 1 papillary carcinoma (4%) and 2 nodules of Hashimoto´s thyroiditis. Mortality in our series was 0. 4 cases of transitory hypocalcemia were observed. Conclusions: The surgeries performed were the total thyroidectomy, the lobectomy and the isthmectomy. A total thyroidectomy was performed for the treatment of the goiters and the thyroid nodules with possible malignity. A lobectomy and isthmectomy for the treatment of thyroid nodules with very low suspicious of malignancy. Objetivos: Se estudia una serie de pacientes portadores de patología tiroidea y su tratamiento. Material y métodos: Son 128 pacientes y 132 cirugías, con 4 reintervenciones. Edad promedio de 55 años; 87 mujeres y 41 hombres. Se realizaron 105 tiroidectomías totales (80%). 60 casos (45%) de bocios, 41 (31%) de nódulos tiroideos sospechosos de malignidad y 4 (3%) de reintervenciones para completar una tiroidectomía total. En 27 pacientes (20%) se realizó una lobectomía e istmectomía. Resultados: En los 60 bocios operados, se encontraron 40 nódulos de hiperplasia involución (67%), 12 (20%) tiroiditis de Hashimoto y 8 (13%) bocios parenquimatosos difusos. En 6 casos (10%), se encontraron microcarcinomas papilares. En 41 tiroidectomías totales por nódulos tiroideos, fueron encontrados: 17 adenomas foliculares (41%), 12 carcinomas papilares (29%), 7 carcinomas foliculares (17%), 1 carcinoma medular (2.4%) y otras lesiones. En 27 casos de lobectomía e istmectomía se encontraron: 11 nódulos de hiperplasia (41%), 10 adenomas foliculares (37%), 3 carcinomas foliculares (11%) y 1 carcinoma papilar (4%) y 2 nódulos de tiroiditis de Hashimoto (7%). La mortalidad de la serie fue 0. Se observaron 4 casos de hipocalcemia transitoria. Conclusiones: Las cirugías realizadas fueron la tiroidectomía total y la lobectomía e istmectomía. Se realizó una tiroidectomía total para el tratamiento de los bocios y los nódulos tiroideos sospechosos de malignidad. Se realizó una lobectomía e istmectomía para el tratamiento de los nódulos tiroideos con muy baja sospecha de malignidad. La mortalidad de la serie fue cero y la morbilidad baja. 97 pacientes tuvieron lesiones benignas (76%) y 31 malignas (24%). Palabras claves: cirugía tiroidea, tiroidectomías.
