Examination and treatment of lithiasis of the main bile duct in laparoscopic surgery
Exploración y tratamiento de la litiasis de la vía biliar principal en cirugía laparoscópica
Resumen:
by means of videosurgery is one of the presentchallenges far laparoscopic biliar surgery. The authorsanalyse the incidence of lithiasis of the MBV in a series of425 laparoscopic cholecystectomies carried out at theHospital Italiano and Hospital Español in Montevideobetween May 1991 and May 1994 and present theirstudy and treatment in the pre, intra and postoperativephases. They discuss the use of retrograde endoscopiccholangiopancreatography and transcysticcholangiography. They a/so present differentlaparoscopic techniques far the extrae/ion of choledocianstones and propase an algorythm far stydy andtreatment. The authors believe that future tendencies wi/1consist of laparoscopic treatment of biliar lithiasis (gal/bladder and choledocian) in a single stage whereintraoperative echography, transcystic extrae/ion ofcholedocian stones and transcystic anterogradepapillotomy wi/1 possibly have an importan/ place.
El manejo de la litiasis de la vfa biliar principal (VBP) porvideocirugfa constituye uno de los actuales desaffos dela cirugfa biliar laparoscópica. Se analiza la incidencia delitiasis de la VBP en una serie de 425 colecistectomfaslaparoscópicas (CL) realizadas en el Hospital Italiano yHospital Español de Montevideo en el perfodo mayo1991 a mayo 1994, presentando su forma de estudio ytratamiento en la fase pre, intra y posoperatoria. Sediscute el uso de la colangiopancreatograffa endoscópicaretrógrada (CPER) y de la colangiograffa trancfstica. Sepresentan las distintas técnicas laparoscópicas para laextracción de cálculos coledocianos proponiendo unalgoritmo para su estudio y tratamiento.Se plantea como tendencia futura, el tratamientolaparoscópico de la litiasis biliar (vesicular y coledociana)en una sola etapa; en la cual posiblemente la ecografíaintraoperatoria, la extracción trancfstica de cálculoscoledocianos y la papilotomfa anterógrada trancfsticajugarán un importante papel.
