Resumen:

This is an 89-year-old male patient, with a mitral prosthesis, who underwent surgery 20 years ago, and came to the emergency room due to abdominal pain and vomiting. A thoraco-abdominal computed tomography (CT) was performed, which confirmed the presence of subxiphoid eventration with parietal distress, requiring intestinal resection and eventhroplasty with intraperitoneal mesh. Subxiphoid eventration is a rare pathology, which mostly occurs in patients who have required interventions in the cardiac or hepatobiliopancreatic area. Its diagnosis is based on clinical symptoms and physical examination. This pathology represents a challenge for the surgeon given the surgical complexity of the anatomical region where it appears, since the adjacent chondrocostal structures make suturing and eventhroplasty difficult. In almost all series the repair has been selective, with emergency surgery due to incarceration being something anecdotal. The suggested technique would be preperitoneal eventhroplasty with non-absorbable retroxiphoid mesh; however, this is conditional on intraoperative findings, especially strangulation.


Se trata de un paciente de sexo masculino de 89 años de edad, portador de prótesis mitral, intervenido hace 20 años, acude a urgencias por dolor abdominal y vómitos. Se realiza una tomografía computarizada (TC) toraco-abdominal que confirma la presencia de eventración subxifoidea con sufrimiento parietal, requiriendo resección intestinal y eventroplastia con malla intraperitoneal. La eventración subxifoidea constituye una patología infrecuente, que tiene lugar, de forma mayoritaria, en pacientes que han precisado intervenciones del área cardíaca, o hepatobiliopancreática. Su diagnóstico se basa en la clínica y examen físico. Esta patología supone un reto para el cirujano dada la complejidad quirúrgica de la región anatómica donde aparece, pues las estructuras condrocostales adyacentes dificultan la sutura y eventroplastia. En casi todas las series la reparación ha sido electiva, siendo la cirugía de urgencia por incarceración algo anecdótico. La técnica sugerida sería la eventroplastia preperitoneal con malla retroxifoidea irreabsorbible, sin embargo, ésta se encuentra condicionada a los hallazgos intraoperatorios, especialmente a la estrangulación.


Trata-se de um paciente do sexo masculino, 89 anos, portador de prótese mitral, submetido a cirurgia há 20 anos, que procurou o pronto-socorro por dor abdominal e vômitos. Foi realizada tomografia computadorizada (TC) toracoabdominal que confirmou a presença de eventração subxifóide com sofrimento parietal, necessitando de ressecção intestinal e eventroplastia com tela intraperitoneal. A eventração subxifóide é uma patologia rara, que ocorre principalmente em pacientes que necessitaram de intervenções na área cardíaca ou hepatobiliopancreática. Seu diagnóstico é baseado nos sintomas clínicos e no exame físico. Esta patologia representa um desafio para o cirurgião dada a complexidade cirúrgica da região anatômica onde surge, uma vez que as estruturas condrocostais adjacentes dificultam a sutura e a eventroplastia. Em quase todas as séries o reparo foi seletivo, sendo a cirurgia de emergência por encarceramento algo anedótico. A técnica sugerida seria a eventroplastia pré-peritoneal com tela retroxifóide inabsorvível, porém condicionada aos achados intraoperatórios, principalmente estrangulamento;
