Resumen:
Diaphragmatic rupture is a pathological condition which is becoming more and more frequent due to automobile accidents. As a rule this occurs in seriouslypolitraumatized patients, with considerable cardiorespiratory distress. These are injuries where diagnosis is difficult and the clinical definition of the lesion is of paramountimportance. The authors, referring to their experience of four cases, explained the etiopathogenic factors, clinical evolutio,_ and therapy applied. If necessary thoraxic surgeryshould be performed. The election of the method of approch depends on the diagnostic localization, Abdominal approach is recommended when associated intra-peritoneal lesions Rre suspected. In the absence of intra-abdominal injury, but with diagnosis of probable diaphragmatic rupture, a thoracotomy is recommended. The closure of the diaphragm should be very carPfully ne with non-absorbable elements. The post operatory management should be the responsibility of the same team that began cardiocirculatory recuperation and perf ,rmed the operatior..
La ruptura diafragmática es una entidad patológica en evidente aumento, incrementada por los accidentes automovilísticos. Generalmente se halla asociada a unpolitraumatizado grave, con importantes trastornos cardiorrespiratorios. Son lesiones de difícil diagnóstico, donde la sospecha clínica de la lesión es el factor principal. Basados en una corta experiencia de 4 casos los autores estudian someramente los factores etiopatogénicos, la evolución clínica y la conducta terapéutica.Lo solución quirúrgica es torácica. La elección de la vía de abordaje varía con el momento del diagnóstico. El abordaje abdominal es aconcejable cuando se sospechan lesiones intraperitoneales asociadas. En ausencia de lesiones intrabdominales y con el diagnóstico previo de RD, el abordaje por toracotomía es el preferido. El cierre diafragmático debe ser realizado de manera cuidadosa con elementos no reabsorbibles. El manejo postoperatorio debe ser continuado por el mismo equipo que comenzó la recuperación card.iocirculatoriay permitió el acto operatorio.
