Afecciones biliares agudas en geriatría: análisis de 151 observaciones , conceptos clínicos y diagnósticos
Resumen:
The authors performed a diagnostic review of 151 paticnts older than 65 who presented non-neoplasic acute biliary processes. The most frequent clinical findingswere discussed , pointing out: the poor simptomatology reiated to the magnitude of the surgical findings; the high nccurrence of signs of participation of the commonbile duct (2 0 % of choledocholitiasis), the frequent eariy peritoneal involvement (9 % of the cases), so frequently ignored ( 7 cases out of 16). and the high incidence of associated affections (s pecially cardiovascula r ailments and d;abetes), which compel to an early surgery in order to correct the imbalance the acute biliary affection means far them.
Se reaJfaa una revisión diagnóstica de 151 enfermos mayores de 65 a,ños, con cuadros agudos biliares de origen no neoplásico. Se analizanl-as manifesta.ciones clínkas más frecuentes, destacándose: la pobreza sintomatológica en relación a 1-a ma.grnitud de los ha.Uazgos operatorios; la alta incidencia de signc,s de participación de la vía biUar pri?cipal (20 % de· litiasis cole-dociana); la frecue1ncia de complicaciones neritoneales tempranas (9 % de la serie), que son a menudo ignoraidas (7 casos en 16); '9 la alta incidencia de afecciones asociadas (sobre tod.o cardiovasculares y diabetes), que. ·obligan a un tratamieil!to operatorio precoz y oportuno,para corregir la descompensación que la afección biliar aguda significa para ellas.
