Chemotherapy in "oat cell" carcinoma of the lung: PRELIMINARY RESULTS OF TREATMENT WITH A POLYCHEMOTHERAPY SCHEME OF CYTOKINETIC BASES

Quimioterapia en el carcinoma "oat cell" del pulmón: RESULTADOS PRELIMINARES DEL TRATAMIENTO CON UN ESQUEMA DE POLIQUIMIOTERAPIA DE FUNDAMENTOS CITOCINETICOS

Resumen:

A preliminary evaluation of the therapeutical results and the tolerance achieved in eight patients with "oat cell" carcinoma of the lung treated with a intennitent scheme of chemoimm \lnotherapy consisting of five drugs -CFM, MTX, ADR, CCNU and Parvum: "EMACC"-, is reported. Under this treatment it was obtained a tumoral remission rate of 5/8 (62.5 %) ; in seven patients it was found an improvement in the perfo-anance status (87.5%), and after seven months of follow-up, 61.5% of the patients are alive, with an acceptable clinical tolerance but a high toxic haematologic cost.Different considerations about the biological behaviour of the disease and the rational way to focus its treatment are made, the cytocinetic basis on the design of the reported scheme are established and it is remarked the non merggence of encephalic metastases through the treatment time. This last fact is related to the chemoprophylactic effect of CCNU on the C.N.S.


Se realiza una evaluación preliminar de los resultados terapéuticos y la tolerancia obtenidos en 8 pacientes portadores de carcinoma "oat cell" pulmonar tratados con un esquema de quimioinmunoterapia intermitente constituí do por 5 drogas-CFM, MTX, ADR, CCNU y C. Parvum; "EMACC"-, registrándose un índice de remisiones tumorales de 5/8 (62.5%), una mejoría subjetiva en 7 casos (87.5%) y una sobrevida actuarial a los 7 meses de seguimiento de 61.5% de los pacientes, con una aceptable tolerancia clínica pero con un alto costo tóxico hematológico. Se hacen consideraciones sobre el comportamiento biológico de esta enfermedad y el encare racional de su tratamiento, se exponen los fundamentos citocinéticos de la confección de dicho esquema y se destaca finalmente la no aparición de metástasis encefálicas en el curso del tratamiento, relacionándose este hecho con el efecto quimioprofiláctico del CCNU sobre el S.N .C.
