Parálisis recurrenciales quirúrgicas
Resumen:
The authors base their experience. on 57 patients with laryngeal paralysis, studied during a three-year period. They review the functional anatomy of the larynx insisting upan its innervation and circulation.Its very rich vascular net is closely related to the circulation of the thyroid gland. The authors experimental studies prove the particular characteristics of the laryngeal muscles and their early damage on anoxia. They insist upon the spirometric tests, an objetive indirect method used to measure glotic sufficiency. The medical treatment and the different surgical techniques used are discussed. 11 patients were operated upan: 9 of them with bilateral paralysis, in which the best results were obtained with the lateral approach and 2 with unilateral paralysis, both of them treated by cartilage graft through anterior route, with satisfactory results.
Los autores asan su experiencia en 57 enfermos portadores de parálisis laríngas, estudiados en un lapso de 3 años. Se repasa la anatomía funcional de la laringe, insistiendo en suinervación y circulación. Su red vascuar es muy rica y está en íntima, relación con la de la glándula tiroides. Estudios histoquímicos de los autores demuestran las características particulares de los músculos laríngeos y su precozdeterioro frente a la anoxia. Se insiste en la importancia de los estudios espirométrioos, método indirecto y objetivo, usado por los autores para medir la suficiencia glótica. Se analiza el tratamieno médico y las diferentes técnicas quirúrgicas utilizadas, jerarquizando la importancia que tiene en todas ellas, el uso del microscopio de operaciones. Se intervinieron 11 enfermos, 9 con parálisis recurrencial bilateral, en las que los mejores resultados se obtuvieron con el abordaje lateral y 2 parálisis unilaterales, ambas tratadas con injerto de artílago por vía anterior, con un buen resultado.
