The surgical treatment of ascending aortic aneurysms associated with aortic valve disease

El tratamiento quirúrgico de los aneurismas de aorta ascendente asociados a valvulopatía aórtica

Resumen:

The authors present the experience achieved at the Heart Surgery Department of the "Clínicas Hospital" with patients with aneurysm of the ascending aorta and aortic insufficiency.A patient was surgically explored and died suddenly while awaiting his reoperation.A patient who underwent valvular replacement and aneurusmoraffia, died in the postoperative for the strong deterioration of his myocardial function.A patient was operated on replacing his aortic valve and the ascending aorta in its supracoronary portian, protecting him with cardiac paralysis; the near and distant evolution was good.It is currently considered that these patients are to be surgically treated in our enviroment. The surgical technique will depend upon the degree of involvement of the aortic wall in each particular case.The valvular prostheses will be mechanical; since a high rate of dysfunction has been observed with the use of some biological valves in our enviroment. A periodical control with aortography is recoended for an early recognition of the postoperative local complications.


Se presenta la experiencia del Departamento de Cirugía Cardíaca del Hospital de Clínicas con pacientes portadores de aneurisma de aorta ascendente de insuficiencia aórtica.Un paciente fue explorado quirúrgicamente y falleció en forma brusca mientras esperaba su reoperación.Un paciente fue sometido a reemplazo valvular y aneurismorrafia; falleció en el postoperatorio por deterioro marcado de la función miocárdica. Un paciente fue operado reemplazándose la válvula aórtica y el aneurisma de la aorta ascendente en su sector supracoronario, protegiendo con cardioplejia: la evolución inmediata y alejada fue buena.Se considera que éstos pacientes son de tratamiento quirúrgico en la actualidad en nuestro medio.La técnica quirúrgica dependerá del compromiso de la pared aórtica en cada caso las prótesis valvulares serán mecánicas, dado el alto índice de disfunción que se han detectado con algunas válvulas biológicas en nuestro medio.Se recomienda el control periódico con aortografía, para detectar precozmente complicaciones locales postoperatorias.
