Traumatic pneumothorax and hemothorax
Neumotórax y hemotórax traumáticos
Resumen:
To understand the physiopathology of pleural occupation, it is necessary to briefly review ali the possible traumatic contingencies. which may be present in thoraxic contusions. Based on the study of 45 patients with serious injuries, 95 % of which presented abnormal pleural occupation, the authors explained the therapy app!i.ed. This depends on the general condition of the patient, of the possible consequences to the injured party and the magnitude of the hemorrhage. An hemopneumothorax is, in itself, a systematic indication of intercostal drainage_ But simple pneumothorax and hemothorax will depend on the degree of their extension. The technical methods used to perform drainage are explained. Emphasis is placed on the manner in which thoracotomy shiuld be performed in traumatized patients, and the different forms of approach are described.
El conocimiento de la fisiopatologia de las ocupaciones pleurales implica necesariamente un breve repaso de todas las posibles contingencias traumáticas que puede presentar las contusiones torácicas. Basados en una serie de 45 traumatizados graves, de_ los cuales el 95 % presentaron ocupaciones pleurales anormales, los autores plantean sus conductas terapéuticas. Ellas parten del estado general del paciente, de las posibilidades ulteriores del traumatizado y de las magnitudes de los derrames presentes. El hemoneumotóraxes de por sí indicación sistemática de drenaje intercostal. Pero los neumotórax y hemotórax puros dependerán del grado de extensión de los mismos. Se insiste además en los procedimientos técnicos para la colocación del drenaje intercostal. Finalmente se recalcan las indicaciones esenciales para la realización de toracotomía en los traumatizadosy las vías de abordaje utilizadas.
