Resumen:
Fifty cases of upper gastrointestinal haemorrhage in geriatric ulcer patients ( older than 65) are discussed.Fourteen cases were given only medica! treatment, with a mortality rate of 50 % . Thirty six of them were operated: 24 as an emergency procedure, with 6 deaths ( 25 % ) , and 12 had a delayed operation, with only one death. Gastrectomy has a high morbility and a 50 %mortality rate in emergency; this drops to 11 % when operation is delayed. Advisability and opportunity ofthe surgical treatment represent a great responsibility for ,the treating team. Minor or em·ergency procedures may be advisable in severa! cases.
Se analizan 50 observaciones, de hemorragia digestiva alta por úlcera gastroduodenal en pacientesmayores de 65 años. En 14 casos se realizó tratamiento médico exclusivo, con 50 % de mortalidad. 36 casos fueron operados: 24 de urgencia,con 6 muertes (25 % ) y 12 en forma diferida, con una sola muerte. La gastrectomía tiene una elevada morbilidad y un 50 % de mortalida,d en la urgencia; sólo un 11 % cuando se realiza en forma diferida. La indicación y oportunillad del tratamiento quirúrgico son una gran responsabilid·ad para el equipo tratante. Los pr,ocedimientos menores o de emergencia, pueden tener i94't ciones en muchos casos.
