Resumen:
The malignant melanomas of the skin can appear in melanoblasts of seemingly normal skin, in melanotic precancerous scales of the Dubreuilh-Hutchinson type,or in junction nevus. These develop from a precancerous stage to a state of melanoma in situ, and then t0 a form of infiltrating melanoma.There are two forms of evolution: one slow of superficial localization, and the other quick with deep infiltration in adjacent tissues. Also three forms of anatomopathological tumors:1) superficial infiltrating melanomas or originated by premelanotic melanosis; 2) melanomas of lentigomelanosis or lentigo malignant origin; 3) nodular melanomas.Deep infiltration affects the process and diagnosis of melanoma. Five different degrees are established according to the depth of cutaneous peentration.Lymphatic diffusion has gr.eat importance in the evolution, and through this satel!itosis and lymph nodes invasion is produced. Perfusion treatments destroy the so-called in transitmetastases: and under these circumstances Alkeran has proved to be very effective in sone of our cases.
Los melanomas malignos cutáneos pueden originarse en los melanoblastos de la piel aparentemente normal, en placas melanóticas precancerosas del tipoDubreuilh-Hutchinson o en un nevus de unión. Evolucionan de un estado precanceroso a un estado de melanoma "in situ"' a una forma de melanoma infiltrante.Existen 2 formas de evolución: una lenta de localización superficial y una rápida con infiltración profunda de los tejidos vecinos. Existen 3 tipos de melanomas malignos: 1) Melanomas infiltrantes superficiales u originados en melanosis premelanóticas; 2) melanomas originados en pecas melánicas o en Lentigo Malignas, y; 3) melanomasnodulares. La infiltración en profundidad condiciona la evolución y el pronóstico del melanoma. Existen 5 grados establecidos de acuerdo a la profundidad de penetración cutánea. La difusión linfática es elemento de gran importancia en la evolución, se establece media nte ella la satelitósis y la invasión ganglionar.Los tratamientos mediante perfusión tienden a destruir estas metástasis llamadas en tránsito; en estas circunstancias el Alkerán ha demostrado ser de gran eficacia en algunos casos estudiados por nosotros.
