Tratamiento de la litiasis coledociana por vía laparoscópica, serie de casos de la Clínica Quirúrgica 2 del Hospital Maciel

Resumen:

This paper presents the results achieved with patientstreated for lithiasis of common bife duct withinthe /ast five years, at the Central Hospital of theArmed Forces.Case material invo/ves 100 cases, 77 of which wereoperated through laparotomy and 23 through laparoscopíc surgery Wíthín the group of those operatedby laparotomy, 40 were treated through a choledochotomy,a choledocho-/ítotomy and draínagethrough a Kehr tube; in 18 cases ca/cutí were extractedthrough the trans-cystíc path. Fífteen choledochoduodenostomíes,1 hepatojejunostomy, 2transduodenal surgícal papillotomíes were performedand 1 patíent underwent left liver resection.Within the group of those treated through laparoscopícsurgery, 15 cases were resolved through thetranscystíc path and 2 through cho/edochotomySix conversíons were performed and there were 2complícatíons (a bílioma and a residual líthiasis).Twenty-cases of CPER wíth papíl/otomy, were performedwhether independently or combined withsurgery.


En este trabajo se muestran los resultados de lospacientes tratados por litiasis coledociana en losúltimos cinco años, en el Hospital Central de lasFFAA.Se trata de 100 casos de los cuales 77 se operaronpor laparotomía y 23 por cirugía laparoscópica.En el grupo de los operados por laparotomía, 40fueron tratados mediante la realización de una coledocotomía,co/edoco/itotomía y drenaje con tubode Kehr; en 18 casos los cálculos fueron extraídospor vía trancística. Se realizaron 15 coledocoduodenostomías,una hepaticoyeyunostomía, 2 papilotomíasquirúrgicas transduodenales y a un pacientese le realizó una resección hepática izquierda.
