El polo superior de la glándula tiroides: el manejo quirúrgico
Resumen:
The author comments on anatomical characteristics of superior pole of thyroid gland and its incidence onsurgical technique during the course of thyroidectomies. Surgical steps in the management of the superior pole of thyroid are analyzed. They are: exposure, disinhibition, hemostasis of superior pedicle and descent of pole. Adequate manouvres for each step are considered, with stress on anatomical knowledge and its application during surgery. Risk involved in such manouvres is mentioned; and exhaustively analyzed; possibility of haemorrhage, nerve aggression, visceral lesions and aggression of parathyroid glands.
El autor hace consideraciones sobre las características anatómicas del polo superior de la glándula tiroides y sus proyecciones a la técnica quirúrgica en elcurso de la tiroidectomía. Se analizan los tiempos quirúrgicos en el manejo del polo superior de la tiroides. Ellos comprenden la exposición, liberación, hemostasis del pedículo superior y el descenso polar. En cada uno de estos tiempos se consideran las maniobras adecuadas para su realización, haciéndose destacar el valor del conocimiento anatómico y su utilización en el curso de la técnica. Se hace mención a los distintos riesgos que existen en estas maniobras, analizándose en todos sus aspectos la posibilidad de la hemorragia, la agresión nerviosa, ll lesión visceral y la agresión de las glándulas paratiroides.
