Neurofibromatosis of head of pancreas: Obstructive jaundice
Neurofibromatosis de cabeza de páncreas: Ictericia obstructiva
Resumen:
A 43-year-old patient from the provinces was suffering from Recklinghausen's disease with a 2-monthevolution cold obstructive jaundice. Paraclinic preoperative exploration showed complete biliary extrahepatic obstruction and considerable dilatation of intrahepatic biliary tract clearly shown by studies performed by conventional scintillogTaphic methods and by Bengal Rose. These methods of study are of considerable importance. Surgical e.xploration showed a tumor in the head of the pancreas which compressed the intra pancreatic biliary duct and diffuse peritoneal tumoral formations which histol'ogy defines as Neurofibromas. Cholecystojejunostomy was performed and immediate evolution was good, distant results are unknown. The papercontains a review of literature which indicates how rare this location is, since ours seems to be the first case of pancreatic location with obstructive jaundice.There follows a discussion of evolutive possibilities of this disease, with special emphasis on the risk of its becoming malign; the conclusion is that whenever possible the ideal treatment in such cases is ressection.
Hemos presentado el caso de un paciente de 43 años, procedente del interior del país, portador de una neurofibromatosis de Recklinghausen, que presenta una ictericia fria, obstructiva de 2 meses de evolución donde la exploración paraclínica preoperatoria muestra una obstrucción biliar completa, extrahepática, con gran dilatación de la vía biliar intrahepática puesta de manifiesto por los estudios centellográficos convencionales y con Rosa de Bengala. Insistimos sobre la importancia de estos métodos de estudios. La exploración quirúrgica revela tumor de cabeza de páncreas con compresión de la v b p y formaciones tumorales pritoneales difusas que la A.P define como neurofibromas. Se trata mediante colecisto-yeyunostomía, siendo buena la evolución inmediata desconociéndose la alejada.Efectuamos revisión bilbiográfica que muestra lo excepcional de esta localización siendo el nuestro al parecer el primer caso de localización pancreática e ictericia obstructiva.Hacemos consideraciones acerca de las posilibilidades evolutivas insistiendo sobre los riesgos de transformación maligna y concluyéndose que de ser posible la conducta ideal en casos similadres es la resección.
