The value of radiation therapy in the treatment of W ilms tumor
El valor de la radioterapia en el tratamiento del tumor de W ilms
Resumen:
A retrospective analysis is presented on 51 cases of Wilms' tumor seen 1953 through 1969. No survivors were seen in a group of 12 cases with inoperable or me.tastatic tumors treated with incomplete surgical resection or radiotherapy only. Nine out of 20 cases (54 % ) survived after nephrectomy and postoperative irradiation, whilst 11 out of 14 cases ( 79 % ) survivedafter preoperative irradiation, nephrectomy and in sone cases postoperative irradiation. Survival in resectable cases under the age of 2 years was 78 %, and 41 %for cases over the age of 2 years. Out of 14 cases treated with the association of Actinomycin D 64 % survived, whereas 71 % survive_d in a comparable group of 14 cases where the drug was omited. Pulmonary prophylactic irradiation seems to have improved the cure rate: out of 11 cases so treated 82 % survived, whilst out of 17 cases whose lungs were not irradiatedprophylactically, 59 % survived.
Estudio retrospectivo de la evolución de 51 casos de tumor de Wilms vistos entre 1953 y 1969. Ningún caso de 12 pacientes con tumores metastásicos o con op. eraciones incompletas ha sobrevivido. Uno en cinco curó con cirugía solamente. Nueve en 20 casos ( 45 % ) tratados con nefroctomía e irradiación postoperatoria sobrevivieron frente a 11 de 24 casos (79 % ) en que se· efectuó irradiación preoperatoria, nefrectomía y en alguno de ellos irradiación postoperatoria. La curabilidad en los casos resecables fue de 7l % para losmenores de 2 años de edad y del 41 % para los mayores de. 2 años. En 14 casos tratados con Actinomicina D asociada la sobrevida fue del 64% frente a 71 % para otros 14 casos comparables en los que la droga no fue· empleada. La irradiación pulmonar profiláctica parece haber mejorado los resultados: en 11 casos en que fue empleada curaron el 82 % , frente al 59 % de curaciones en 1 7 casos en que no fue empleada.
