Tratamiento quirúrgico del pie plano

Resumen:

In children between the ages of eight and fourteen, in whom arch supports have failed, a surgical therapy is suggested. It is important to restore heel alignment,correcting its va!gus and pronation (interna! rotation), is well to correct the longitudinal arch. The operation is performed on tendons and capsular structures, without any arthrodesis. The lengthening of the tendo calcaneous is a preliminary procedure aimed at freeing the heel for its rea!ignment. The anterior tibial tendon is transferred, rerouting it without disinsertion and passing it through an osteotomy and a slot in the navicular end; the posterior tibia! tendon which had previously been dissected from its insertion, is reattached.For eight weeks a cast is applied. Walking is not permitted until then. 


Presentamos una terapéutica quirúrgica del pie plano en nmos entre 8 y 14 años de edad en los cuales ha fa11ado el uso de plantares. El autor destaca la importancia de restaurar elalineamiento del retropié corrigiendo su valgo y la pronación además de 1a corrección del arco longitudinal. La operación se hace sobre elementos tendinososy capsulares, sin artrodesis. En primer término se hace el alargamiento del tendón de Aquiles, para liberar el retropié y poderhacer así su realineamiento. Se hace trasplante del tibial anterior sin desinsertarlo, dándole un nuevo trayecto pasándolo por una osteotomía y perforación en el escafoides, asícomo la reinserción del tibial posterior, disecado anteriormente. Se hace yeso por 8 semanas. No se permite el apoyo hasta cumplir ese plazo
