Resumen:

The authors report on a case which is interesting because of its rare occurence, although it is advisable to be on the alert for its presence. Barium fecaloma which is retained for a long time in the recto-sigmoid will eventually be the cause of perforation due to decubitus ulcer as it rests on nonextensible surrounding tissues such as the pelvic wallFecaloid peritonitis develops and rapidly kills the patinet.This case was favourably resolved by resection of a recto-sigmoid segment which included the barium fecaloma, closure of distal end and left iliac colostomy. Similar cases published in world literature are rare and this has lead its authors to publish this new case, the first to be known in this country.


Se presenta un caso de interés. por su baja frecuencia, pero sobre el que debemos estar siempre prevenidos.El baritoma detenido por mucho tiempo en el recto-sigmoides va a produce una perforación por úlcera de decúbito al apoyar sobre tejidos vecinos inextensibles como lo es la pared pelviana.La peritonitis fecaloides que sobreviene, mata rápidamente al paciente.Este caso fue resuelto favorablemente, resecando un segmento de recto-sigmoides que incluye el baritoma, cierre del cabo distal y colostomía ilíaca izquierda.Los casos similares publicados en la literatura mundial son muy escasos, lo que motiva la presentación de este nuevo caso, el primero conocido en nuestro país.
