Surgical practice in Uruguay during the COVID-19 Pandemic: Analysis of qualitative and quantitative aspects of its initial phase.

Práctica quirúrgica en Uruguay durante la Pandemia COVID-19: Análisis de aspectos cualitativos y cuantitativos de su fase inicial.

Resumen:

Due to the SARS-CoV-2 infection pandemic in Uruguay, guidelines and recommendations were established that radically modified the usual surgical procedures. These measures were based on recommendations based on previous international experience, tending to perform only urgent / emergency surgeries, oncological patients and those who cannot be postponed. Objective. Analyze the impact of the COVID-19 pandemic on the practice of surgical specialists in our country. Material and methods. A survey was carried out and disseminated through the scientific surgical societies of Uruguay, which was kept open for a period of 10 days (April 20 to 30, 2020), collecting a total of 262 responses, of which 212 were complete. . Results: 52% of those surveyed performed scheduled surgeries and 55% performed video-assisted approaches; 51% of those who performed surgeries denied having taken additional protective measures during them. Twenty-three (9%) of the specialists operated on patients suspected of carrying Covid. 19 and 3 (1.1%) positive COVID-19 cases. 57% considered themselves "partially trained" and 21% "poorly trained" (21%), while 19% defined themselves as "fully trained" for the use of protection elements. Conclusions: High compliance with the suspension measures for coordination surgeries was verified (decrease of 82). Surgeries in patients with COVID-19 were exceptional and the frequency of cases with suspected disease was low. The excess of information and the existence of multiple protocols have determined considerable heterogeneity in the implementation of protection measures and low participation in training activities.


A causa de la pandemia por infección SARS-CoV-2  en Uruguay, se  establecieron  pautas y recomendaciones  que modificaron radicalmente las prestaciones quirúrgicas habituales. Estas medidas se basaron en recomendaciones fundadas en la experiencia internacional previa, tendientes a realizar solamente cirugías de urgencia/emergencia, pacientes oncológicos e impostergables. Objetivo. Analizar el impacto de  la pandemia COVID-19 en la práctica de los especialistas quirúrgicos en nuestro país.  Material y métodos. Se realizó una encuesta que fue difundida a través de las sociedades científicas quirúrgicas de Uruguay, que se mantuvo abierta por un periodo de 10 días (20 al 30 de abril de 2020), recabándose un total de 262 respuestas, de las cuales 212 fueron completas. Resultados: El 52% de los encuestados realizo cirugías programadas y el 55% realizo abordajes video-asistidos; el 51% de quienes practicaron cirugías negó haber tomado medidas de protección adicionales durante las mismas. Veintitrés (9%) de los especialistas operaron  pacientes sospechosos de portar Covid.19 y 3 (1.1 %) casos positivos COVID-19. El 57% se consideró “parcialmente capacitado” y 21%  “poco capacitado” (21%), mientras que el 19 % se definió como “totalmente capacitado” para la utilización de los elementos de protección. Conclusiones: Se verifico  alto  acatamiento a las medidas de  suspensión de las cirugías de coordinación (descenso de 82). Las cirugías en pacientes cursando COVID-19 fue excepcional y la frecuencia  de casos  con sospecha de enfermedad fue  baja. El exceso de información y la existencia de múltiples protocolos han determinado una considerable heterogeneidad en la implementación de las medidas de protección y baja participación  en  actividades de capacitación.
