Hemoptisis masiva (H. M.)
Resumen:
The author presents a case of massive hemoptysis which determined a surgical treatment for a double lesion: 1) Retention of an hydatic membrane in the upper left lobe which was treated with an atypical resection. 2) Lower lobe spoiled by bronchiectasis and chronic pneumonitis treated with a lobectomy. Eighteen months later the pacient presented a Jobarpneumonia which was solved by a tracheotomy and by a massive antibiotherapy. The principal point of this work is the reaffirmation of Largheros' concepts in 1942 and the revision ofKarlsson's work.
Se presenta 1 caso de H.M. que, obligó a tratar quirúrgi-camente una doble lesión: 1) Retención de membrana hidática de lóbulo superior izquierdo, que fue tratada porresección atípica. 2) Lóbulo inferior ·destruido con bronquiectasias y neumonitis crónica tratado por lobectomía inferior. El paciente a los 18 meses presentó neumoníalobar que se resolvió por traqueotomía y antibioterapia masiva. El encare fundamental es la reafirmación del concepto de Larghero de 1942 y la revisión del trabajo de Karlsson en la conducción terapéutica de estos pacientes.
