Embolic obstruction of the tibio-peroneal trunk and of the tibialis anterior postarteriography with Seldinger catheter: Remote treatment with vein graft
Obstrucción embólica del tronco tibio-peroneo y de la tibial anterior postarteriografía con catéter de Seldinger: Tratamiento alejado con injerto venoso
Resumen:
The authors report a case of embolic obslruction of the tibial-peroneal trunk, following the introduction of a Seldinger's catheter in a healthy femoral artery, in order to study cerebral circulation. The patient, after a severe acute ischemia, was able to preserve the trophism of his limb, although with severe intermitent claudication. An arteriographic study showed complete obstruction of the tibial-peroneal trunk and of the anterior tibial artery in its origin. The treatment consisted on a free graft, uslng the great saphenous vein, from the lower end of the popliteal artery up to the posterior tibia! artery in the middle of the calf, with excellent clinical and arteriographic results.
Se presenta un caso de obstrucción embólica del tronco tibioperoneo luego de la introducción de catéter de Seldinger en arteria femoral sana, para estudio circulatorio cerebral. El enfermo, Juego de un severo cuadro agudo de isquemia, logró mantener el trofismo de su miembro, aunque quedó con severa claudicación intermitente. Un estudio arteriográfico mostró obstrucción completa del tronco tibioperoneo y de la tibial anterior en su origen. El tratamiento consistió en la realización de un injerto libre con vena safena interna desde la arteria poplítea baja a la arteria tibial posterior en parte media de pierna, con excelente resultado clinico y arteriográfico.
