Fatal haemoperitoneum due to ruptured left hepatic artery aneurysm

Resumen:

left hepatic artery, complicated with serious hemoperitoneum caused by the rupture of the aneurysm. I t could be the second case ever to be reported in which the ethiology was acystic degeneration of the medial coat in an artery other than the aorta. The first case reported was also of the hepatic artery in a curious coincidence. The hystology of the area tributary to the thrombosed artery is shown, and proved to be normal. The paper is divided into the following chapters: pathology, clinical study,  diagnosis, treatment. The conditions under which the hepatic artery may be resected or ligated are established. The frequency and the diagnostic difficulties arising from the intrahepatic location of the aneurysm are studied andalso the tactic in front of a hemoperitoneum of non diagnosed cause. 


Se presenta un nuevo caso de aneurisma verdadero de la rama izquierda de la arteria hepática. En la etapa de complicación con hemoperitoneo grave, por rotura del aneurisma.Sería el segundo caso publicado, en que la etiología es una degeneración quística de la capa media, en una arteria que no sea la aorta. El primero también es enla arteria hepática, en curiosa coincidencia. Se presenta el estudio histológico del parénquima hepático dependiente de la arteria trombosada, demostrando su normalidad.Con el análisis de la casuística se hacen los siguientes capítulos: anatomía patológica, síntomas, orientación diagnóstica, orientación terapéutica. Se establecen las condiciones en que esposible ligar o resecar la arteria hepática.  Se analizan las dificultades diagnósticas que crean las topografías intrahiliares, su frecuencia y la conducta a seguir en caso de hemoperitoneos de causa no aparente.
