Early diagnosis of congenital hip dislocation
Diagnóstico precoz de la luxación congénita de la cadera
Resumen:
Diagnosis can be considered "early'.' when established befare the age of six months for the following reasons:1) Beca use congenital thigh displacements constitute progressive lesions and consequently can be treated in the early stages. 2) Because the younger the patient, the faster thegrowth potential. 3) Beca use thus it is possible to start treatment befare the child begins to walk. Diagnosis is established through clinic and radiology. Clinic alerts us, X-rays confirm it. Clinic alerts us through good anamenesis -family history with respect to thigh or other displasias, personal history, breech deliveries, difficulty in separating muscles- andcorrect examination which also evidences other malformations and limitation of muscle abduction or Ortolani's sign. Radiology is of limited value during the first twomonths. Roof obliquity or methaphyseal separation may be caused by physiological factors or defects in technique. When faced with the slightest clinical suspicion a radiological study should be performed between the ages of 2 to 6 months. Clinics and radiology enable us to establish an early diagnosis and start an early treatment.
Consideramos como precoz el diagnóstico hecho antes de los 6 meses de edad. Por tres razones fundamentales: 1) Porque son lesiones progresivas y así las potratar en etapas iniciales.2) Porque el potencial de crecimiento es tanto cuanto menos la edad. 3) Porque hacemos el tratamiento antes del comienzo de la marcha cuando el niño está en brazosde la madre, El diagnóstico es clinico radiológico. La clínica nos alerta, la radiología lo confirma. La clínica nos alerta a través de una buena anamnesis: antecedentesfamiliares de displesia de cadera u otras displaslas, antecedentes personales, parto de nalga, dificultad de separar los muslos y de un correcto examen que nosmuestra otras malformaciones y limitación de la abducción de los muslos o el signo de Ortolani. • La radiología tiene poco valor en los primeros dosmeses de vida. La oblicuidad del techo o la separación de las metáfisis puede ser un hecho fisiológico o defecto de técnica. Ante la menor sospecha clínica se debe tomar unaRx entre el 29 y 69 mes. La clínica y la radiología nos permite hacer un diagnóstico precoz, y por tanto hacer un tratamiento precoz,
