Traumatic injuries of the duodenum
Injurias traumáticas del duodeno
Resumen:
Eighteen cases of traumatic injuries of the. duodenum are discussed. They correspond to a 13-year period in the Emergency Department of the Hospital de Clínicas. This report shows that these lesions have evidently increased in the last few years, due to factorswhich are considered in the discussion. It is remarked how exceptionally pancreatic association was present in this series ( 1 case: 5 % ) . Preoperative diagnosisis difficult, due to severa! circumstances mentioned; it was never done in the cases under consideration. Tbe mechanism, anatomo-clinical types and the therapyused in each case are discussed, insisting upan the severity of the associated lesions and upan the need of a correct surgical exploration .of the abdomen.In 16 cases, a simple closing of the lesion was performed. Evolution was satisfactory in the same percentage of cases. Seventy-two per cent of the patients underw entpost-operative complications: 27 % of them required anuther surgery. Two patients (11 % ) died: one due to puJmonary embolism and another because of associatedlesions. In both cases the duodenal suture, controllec by necropsy, was in good condition.
Se analizan 18 casos de injurias traumáticas del duodeno, correspondientes a un período de 13 años de actuación ,d,el Departamento de Emergenciadel Hospital de Clínicas. Dicho análisis demuestra oue estas lesiones van aumentando en forma evidente en los últimos años, por factoresque se estudian. Uama la atención lo ,excepcional de la asociación pancreática en esta serie (1 caso: 5 % ) . El diagnóstico preoperatorioes difícil, por una serie de circunstancias que se mencionan; no fue hecho nunca en los casos analizados. Se estudia el mec,anismo, los tipos anatomoclínicos y la terapéutica instituida en cada caso, insistiéndose en la gravedad de la,s lesiones as,ociadas y en I,a necesidad de una correcta exploración (!Uirúrgica del abdomen. En 16 casos (87 % ) se efectuó cierre simple de la lesión, siendo la evolución satisfactoria en el mismo porcentaje de casos. El 72 % presentó complioaciones poistoperatorias con 27 % de reintervenciones. Dos fallecieron (11%), uno por una embolia pulmónar y otro por las lesiones asociadas. En ambos casos la sutura duodenal, controlada en I,a necropsia, se hallaba en buenas condiciones.
