Neoplasma gástrico superficial: A propósito de 28 observaciones

Resumen:

The authors analyze 28 cases of superficial gastric neoplasm which had excellent evolution following surgery. They also point out the change in concept because diagnosis should be established, with substitution of the old "clinical - radiologic" criteria for the "clínical histologic radiologic-endoscopic" approach. Evolution of gastric cancer befare it, to an advanced stage is show. Numerous symptoms and signs should induce the physician to suspect the presence of a neoplasm, though in no case should diagnosis be stablished on such basis.Several radiologic complementary techniques are required and double contrast is of considerable value in detecting small lesions or those double contrast detecting small lesions or those located in barely accesible positions where conventional methods fail. Considerable advance has been achieved by fibroendoscopy with directed biopsy, for this method has attained 96 % success in diagnosis of cases studied.


Los autores analizan 28 casos de neoplasma gástrico superficial, con excelente evolución luego diel tratamiento quirúrgico realizado. Señalan la importancia del cambio de concepto en la elaboración dJagnóstica, sustituyendo el antiguo "clínico - radiológico" por el "clínico - radiológico- endoscópico - patológico-". Hacen consideraciones sobre la lentitud evolutiva del cáncer gástrico antes de llegar a etapas avanzadas, y analizan numerosos síntomas y signos que pueden hacer sospechar la presencia del neoplasma, aunque nunca llegar al diagnóstico. Resaltan la importancia de realizar varias técnicas radiológicas complementarias, así como el enorme valor del doble co.ntraste en la detección de lesiones pequeñas o ubicadas en lugares pocoaccesibles para los métodos convencionales. Llaman la atención sobre el gran adelanto que significó la fibroendoscopía con la biopsia dirigida, que llegó a un dfagnóstico certero en un 96 % de los casos estudiados.
