Selective intubation and unilateral ventilation in lung resection
Intubación selectiva y ventilación unilateral en la resección pulmonar
Resumen:
Ten patients with one lung anaesthesia for pulmonary surgery were studied. Gas exchange and Acid-Base equilibrium were evaluated during the procedure. Changes in arterial oxigen tension were never life threatning. When reinhalation circuit were used significant hipercapnia and acidez were detected. Therefore sorne ventilatory pattems are suggested in order to avoid those dangerous phisiological changes.No atelectasia was seen. Surgical advantages obtained with this technique are emphazised.
Se estudiaron 10 pacientes en los que se realizó anestesia con intubación selectiva para cirugía pulmonar.Se analizó la repercusión de esta técnica sobre el intercambio gaseoso y el equilibrio acidobásico. En ningún caso se comprobó tensiones parciales de 02 que comprometieran la seguridad de los pacientes. En casos que se utilizó circuito con reinhalación se detectaron hipercapneas y descensos del pHsignificativos. Se aconseja patrones ventilatorios que eviten esa situación.En ningún caso se comprobó la instalación de atelectasia.Se enfatiza las ventajas de esta técnica al facilitar las maniobras quirúrgicas.
