Peritonitis biliares
Resumen:
The authors define peritonitis originating in the hepatobiliary-pancreatic aparatus and characte-rized by hile in the peritoneum, as biliary peritonitis. They made the following dassification: 1) In the accessory biliary tract: a) by permeation or precocious perforationin case of an obstruction of the excretion route of the gall bladder; b) due to fall of the obstructing gallstone. 2) Of the main biliary tract, pan creas and liver: a) with gall bladder functioning due to filtration or permeation of the gall bladder; b) without a functioni'ng gall bladder (cholecystectomized, cr scleroatrophic gall bladder). These peritonitis are classified according to the location of the obstacle: at the level of the middle part of the gall bladder; in the ampulla of Vater, with a common biliopancreatic route or obstacle of the gall bladder. Pancreatitis lesion_s .Possibility of gall bladder or pancreatic lesions. Each one has a different prognosis and treatment. Thes ones that have the best prognosis are those located in the gallbladder.
Los autores definen como peritonitis biliares a aquellas peritonitis a punto de partida del aparato hepatobiliopancreático que se caracterizan por tener bilis en el peritoneo. Se hace una clasificación de ellas: 1) De la vía biliar accesoria: a) por permeación o perforación precoz en una obstrucción de la vía de excreción vesicular; b) por caída del cálculo obstructor. 2) De la vía biliar principal, el páncreas e hígado: a) con vesícula funcionan te, por filtración o permeac10n vesicular; ·b) sin vesícula funcionante ( colecistectomizado, o vesícula escleroatrófica).Se clasifican estas peritonitis por la 1,,bicación del obstáculo: a nivel del colédoco en su parte media; en la ampolla de Vater, con vía común biliopancreática u obstáculo coledociano. Lesiones de pancreatitis. Posibilidad de lesiones coledocianas o pancre, áticas. Cada una de ellas tiene un pronóstico y tratamiento distinto. Las lesiones de inejor pronóstico son las vesiculares
