Hepatic arteriovenous fistula of traumatic origin
Fístula arteriovenosa hepática de origen traumático
Resumen:
Traumatisms, wounds and contusions of liver may give rise to holes in arterial vessels of varying importance, of branches of the portal vein and of biliaryduet within the hepatic parenchima. Under such circumstances traumatic arteriovenous fistulas may occur and in turn, lead to portal hypertension ( and ascitis in sone cases), hemobilia and hepatic lesions (ischemic, infections or obstruction of biliary ducts), hese may explain hepatic cholics and deterioration of general condition of patients, leading to the patient's death as consequence of these disorders or of hemobilia. Selective arteriography permits diagnosis of this condition which is Hable to occur in any import¡nt !ver traumatism. .Surgery is employed to interrupt communication of this fistula, often through ligature of corresponding arterial or venous vessels and resectionof the hepatic territory tributary to such vessels; less frequently through suture of communication preserving vascular integrity.
Los traumatismos hepáticos, heridas y contusiones, pueden crear soluciones de continuidad de vasos arteriales más o menos importantes, venas ramas de laporta y canales biliares en el seno del parénquima hepático. En estas circunstancias se pueden establecer fístulas arteriovenosas traumáticas, que pueden engendrar una hipertensión portal con ascitis en algunos casos, episodios de hemobilia y lesiones hepáticas isquémicas, infecciosas y de obstrucción canalicular biliar,que explican los cólicos hepáticos y el agravamiento del estado general, que por sí o por las hemobilias pueden conducir a la muerte. El conocimiento de estos hechos lleva a diagnosticar por· la arteriografía selectiva esta situación, que puede aparecer en cualquier traumatismo importante de hígado, y su tratamiento quirúrgico, con el fin de interrumpirla comunicación de esa fístula, a menudo por ligadura de los vasos arteriales y .venosos correspondientes y resección del territorio hepáticb, tributario de esos vasos, menos frecuentemente, por sutura de la comunicación con conservación de la integridad vascular.
