Oleohepatografía
Resumen:
1) The procedure was applied on 28 patientes, 16 of which were females and 12 males ranging between 25 and 71 years of age. In 17 cases splenic percutaneousapproach was employed, in 10 repermeated umbilical approach and in one case the approach was ileal intraoperatory. 2) After determining portal pressure, hydrosolublecontrast was used in detecting possible azygo-portal or porto-cava communications or bivenous shunts (portasuprahepatic), for in this case the method is contraindicatedTwenty ce. of were injected and became apparent three phases: portal, sinusoid and hepatic. The latter had been described by the. author in 1969 from the radiological angle.3) Radiocinematography, performed in 6 patients, brought out new facts of portal hemodynamics, indicating the possibility of measuring circulatory speed. 4) We analyzed the results obtained during the different intra and juxtahepatic expansive processes and the procedure proved to be efficient and inocuous. Two nodules ½ cm. in diameter were observed. It also proved to be efficient in post-operatory control of the adventitions hydatid cavity, as well as in studyirtg local circulatory condition in compensatory hepaticatrophies or hypertrophies. 5) Hystological study 10 patients indicated that the hepatic phase described in radiological observation was present. The lipidic excess in the hepatic celland in Kupfer's cells was observed, as well as its subsequent dispersa! and fragmentation; no other citoarchitectural alterations were observed.
1) Aplicamos el procedimiento a 28 pacientes, 16 del sexo femenino y 12 del sexo masculino cuyas edades variaron entre 25 y 71 años. La vía de administración fue esplénica percutánea en 17 casos, umbilical repermeabilizada en 10 casos y en uno la vía fue ileal intraoperatoria. 2) Previa toma de la presión portal se detecta concontraste hidrosoluble las posibles comunicaciones ázigo-portales o porto-cava o shunts bivenosos (portasuprahepáticos) donde el método está contraindicado.Se inyecta 20 ce. de L.U.F. objetivándose. 3 fases: portal, siriusoidal y hepática esta última descrita por nosotros del punto de vista radiológico en el año 1969.3) La radiocinematografía se realizó en 6 pacientes constatándose nuevos hechos de la hemodinámica portal y la posibilidad de medir la velocidad circulatoria.4) Analizamos los resultados obtenidos en los diferentes procesos expansivos intra y yuxtahepáticos, destacando la eficacia e inocuidad del procedimiento.Pudiendo objetivar en dos casos nódulos de ½ cm. de diámetro. Resultó eficaz en el control post-operatoriode la cavidad hidática adventicia! así como en el estudio del estado circulatorio local en las atrofias e hipertrofias hepáticas compensadoras. 5) El estudio histológico se realizó en 1 O pacientes constatándose la fase. hepática descrita radiológicamente. Se apreció la sobrecarga lipídica de la célula hepática y de las células de Kupfer para luego asistira su dispersión y fragmentación sin estar presentes otras alteraciones cito-arquitecturales.
