Intestinal ischemia in abdominal aortic reconstructive surgery
Isquemia intestinal en la cirugía reconstructiva de la aorta abdominal
Resumen:
Intestinal ischemia is a serious complication of the repairing surgery of the abdominal aorta; generally involving the Jeft colon and more rarely the small intestine or both simultaneously.It is more frequent as a complication of an aneurysmectomy. The incidence of this complication is 0.2 to 10% of the cases. The authors have observed this complication seven times in 216 cases of repairing surgery of the abdominal aorta (3.24%).The global mortality of ischemic colitis is 40%, when the whole intestinal wall is ischemic mortality rises to 90%.5 of the seven cases observed by the authors died (71.42%) The authors discuss the different factors that might cause the development of an intestinal ischemia. The prevention of this complication by means of pre and postoperative care is also discussed.Emphasis is made upon the c!inical features that lead to the correct diagnosis of the established ischemia, and the additional testing methods to certify it.
La isquemia intestinal es una complicación grave de la cirugía reconstructiva de la aorta abdominal, que involucra generalmente el colon izquierdo, más raramente el intestino delgado o ambos simultáneamente.Es más frecuente como complicación de la aneurismectomía.La incidencia de esta complicación se ve del 0.2 al 10% de los casos. Los autores la observaron 7 veces en 216 intervenciones de reconstrucción aórtica (3.24% ). La mortalidad global de la colitis isquémica es del 40%, cuando la isquemia es transmural la mortalidad se eleva al 70% . La isquemia del delgado tiene una mortalidad del 90% . De los 7 casos a los que hacen referencia los autores 5 fallecieron (71.42% ).Los autores discuten los distintos factores que pueden condicionar la instalación de una isquemia intestinal.Se discute la prevención de esta complicación por medidas que deben ser tomadas en el pre- e intraoperatorio.Se destacan los elemento clínicos que permiten realizar el diagnóstico de isquemia constituida, así como las medidas paraclínicas que la certificarán.
