Herida cardíaca por herida de bala en región glútea

Resumen:

A case is described whereby a cardiac wound was caused by a bullet which entered the body through the left gluteal region, and passed through the parietallayers of the abdomen without entering the peritoneal cavity_ This produced an acute hemopericardium due to a transfixiant wound in the lower borderof the right ventricle, which was successfully repaired through a left anterolateral thoracotomy. An immediate laparotomy confirmed the absence of intra-abdominal lesions. Long term evolution was satisfactory. The course followed by the bullet is explained by the position of the patient at the time he received the shot.


Se presenta el caso de una herida de bala cardiopericárdica por un proyectil que entró por la región glútea izquier-da, y recorrió el abdomen por J,os planos parietales, sin atravesar la cavidad peritoncal. Condicionó un hemoperica,r<lio aguP.o, por herida transfixiante de borde inferior de ventrículo derecho, que se logró reparar- exitosamente através de toracotomía anterolatera.J izquierda. Una laparotomía inmediata, confirmó la, ausencia de lesiones intraabdominales. La .evolución alejada fue buena. Se explica esta rara trayectoria, por la posición del paciente en el momento de recibir el impacto.
